Objective:The lumbar interverbral disc protrusion(LIDP for short)is a common and frequently-occurring clinical disease.It is about 15%of the lumbar and leg pain in the clinic,and it severely affect human's work and life.At present,the treatment of this disease mainly includes 3 methods:operation treatment,un-operation treatment and intervention treatment.Operation treatment has risk and syndrome,and intervention treatment also has the less straitness treatment scope and more localization.As a conservative treatment method,Massage has the characteristic that: the wide treatment scope,unique curative effect,less pain,less syndrome,and cheaper cost.Therefore,Most of the sufferers prefers to choose Massage treatment, it has the important social benefit.However,Though there are many reports that massage can treat LIDP in clinic,most of them about ameliorate symptom and signs, and less objectivity index.The mechanism is still not very clear,and blind in clinical treatment.The subject choose such clinical symptom(include symptoms,signs),pain index,IL-1β,hemorheological index as observation index.The objective of Subject is to observe curative effect and mechanism that three steps massage treat LIDP,and provide theoretical basis for clinical treatment.Method:1.Strictly according to the adoption standard and the elimination standard,totally 70 LIDP patients were randomly assigned to a treatment group(35 cases)and a control group(35 cases).All cases were known the fact.The treatment group were treated by three steps massage,and the control group were treated by traction.Treatments were given once a day.30 minutes one time,one time per day,10 days was a course of treatment.After a course of treatment,the patients had two days for a rest.2.Before and after two courses of treatment,clinical symptom was evaluated with Evaluated criteria of lumbar disc recommended by Japanese Orthopaedics and Tranmstology Academy in 1984.It includes three major symptoms(low back pain,pain or numbness of leg,walking ability),five signs(tendemess\radiated pain,straight-leg raising test,sensory disturbance,strength knee or achilles tendon reflex),three daily living abilities(turning around at decubitus\stand up\walking\bending over,prolonged sitting(1 hour),lifting and holding things,bladder function.And the scores were higher,the injury was severer.3.Before and after two courses of treatment,lumbar and leg pain was evaluated with Evaluated criteria of pain index(according to visual analogue score,VAS)4.IL-1βand hemorheological indexes were assayed before and after two courses of treatment.5.All data was handled by statistics SPSS14.0 software package.Measurement data was expressed by sample average±Standard deviation(X±S).The comparation between groups was handled by t testResults:1.Curative effect:In the treatment group,results showed that the total efficient rate was 91.4%.The case with curing,apparent effect,efficient effect and inefficient effect was 8,12,12 and 3 respectively.As to the control group,the total efficient rate was 82.9%.And the case was 5,8,16 and 6 respectively.As to therapeutic effect,there was significant difference between two groups.2.Scores of clinical symptom:there was significant difference in two groups between before and after two courses of treatment(P<0.05),which illustrated two groups all could improve clinical symptoms.And we can also found that scores were decreased after treatment as compared with those before treatment(P<0.05).3.Scores of clinical symptom:there was significant difference in two groups between before and after two courses of treatment(P<0.05),which illustrated two groups all could relieve the pain.And we can also found that scores were decreased after treatment as compared with those before treatment(P<0.05).4.Hemorheological indexes:there was significant difference in two groups between before and after two courses of treatment(P<0.01),which illustrated two groups all could effect the hemorheological indexes.And hemorheological indexes were decreased after treatment as compared with those before treatment(P<0.05). 5.Content of IL-1βin serum:there was significant difference in two groups between before and after two courses of treatment(P<0.05)which illustrated two groups all could effect the content of IL-1β.And content of IL-1βwas decreased after treatment as compared with those before treatment(P<0.05).Conclusion:1.In curative effect,improve clinical symptoms and signs and relieving the pain,three step massage treatment is superior than traction treatment.2.In content of IL-1βand hemorheological indexes,three step massage treatment is superior than traction treatment.And the curative effect with serum IL-1βcontent,hemorheological indexes in line with the downward trend.
