The combination of the Chinese traditional medicine and the western medicine is the common usage in China. The problems induced by this combination, especially the adverse reaction, is becoming more and more important to the researchers in China.This article reviewed the history of the combination, analyzed the mechanisms, the theory and the problems existed , investigated the condition of clinical use and studied the interaction of tannis, anthraquinone and amoxicillin in artificial gastrointestinal juice.Section 1:Literature reviewIntroduced the history and the present use of the combination, the case reports of the interaction, the clinical researches and the experiments on the combination.Section 2:The mechanisms of the interactionWe analyzed the mechanisms of the interaction from the chemical reaction, the change of the acidity and alkalinity, the medicine metabolism and discretion, the antagonism and additive, the production of the toxic substances and the change of internal environment.Section 3:The interaction theory and the questions with itWe introduced the interaction theory of the Chinese medicine and the western medicine which based on the chemical reaction. The theory was questioned by the interaction itself, the affect of the internal environment to the compound and the affect of the dosage form technology to the components, ect.Section 4:The investigations of outpatient prescriptions combinationThe outpatient prescriptions were investigated on the combination of the traditional medicine and the western medicine. The data showed that the combination accounted 76.3% of all the prescriptions. The combination of amoxicillin and the tannis medicine was common. It was reported about the interaction between tannis and penicillin, tannis and the other medicines. There are about 1/4 medicines which contains tannis and antheaquinone in the insurance list 2005.Section 5:The dissolution of Rhizoma Rhei anthraquinone, Galla Chinensis tannis and amoxicillin after the combination in the artificial gastrointestinal juiceThe antraquinone was extracted from Rhizoma Rhei, the tannis was extracted from Galla Chinensis and made them into capsules. The HPLC was used to analyze the dissolution after the combination with amoxixillin in the gastrointestinal juice. The precipitation was produced after the combination of anthraquinone, tannis and amoxicillin. The dissolution of tannis and amoxicillin was reduced. The dissolution of rhein and emodin, the two indications of Rhizoma Rhei, was different in the acid environment and alkaline environment. Comparing with combined and separated, the dissolution of rhein reduced in gastric juice,but increased in intestinal juice; the dissolution of emodin was not affected in the gastric juice, but increased in intestinal juice. The dissolution of amoxicillin reduced which may because of the reaction with the other components undetected. It showed that the absorb of the two components maybe affected by the combination of Rhizoma Rhei anthraquinone, Galla Chinensis tannis and amoxicillin. The precipitation was produced in acid gastric juice but only partial educed in the alkaline intestinal juice.Since the complexity of the internal environment, the experiments in vitro may not be equal with the experiments in vivo. It needs a further study on the interaction of the effective components of the traditional medicine and the western medicines maybe different from the interaction of the extraction of the herbal medicine ,the compound recipe and the dosage form with the western medicine.
