Syndrome, the bridge between TCM theory and clinical practice, is the key of treating principle and prescription, and the core of treatment by differentiation of syndromes (TDS). Syndrome has been considered as the breakthrough of TCM modernization research for many years. The research of syndrome essence is the major subject. The essence and objective indexes of syndrome has been probed without adopting the concept of modern diseases. Essence of syndrome could be interpreted and measured objectively if some special constituents could be discovered. Then, it is possible to integrate traditional Chinese medicine with western medicine in essence. Since 1950, syndrome essence has been the key target of the TCM modernization. Breakthroughs were expected, but the results were disappointing.We found there were many problems in the research of TCM syndrome concepts and the essence research by summarizing and analyzing the literature. In the research of TCM syndrome concepts, there are too many problems, such as obscurities in definition, confusion between syndromes and symptoms, and unsuitable methodology. The major problems are summarized as follows: first, it was segmentary in research way. The reducible and analytical linear methods were adopted to discuss syndrome essence. The methods,which dissevered a whole to parts,had great limitations. Second, some special indexes were denied, and new special indexes with significance of theoretical guidance and clinical value had not been obtained in the experiment study. On the contrary, there are many indexes with the same changing trend among different syndromes. The research results didn't guide the clinical practice and improve the clinical curative effect.Treatment by differentiation of syndromes (TDS) is a primary method to diagnose diseases based on the theory of TCM. Though there are simple recipes and proved recipes, there is no doubt that treatment by differentiation of syndromes (TDS) is the dominant method. Diagnosis, theory and medication, as a whole, are included in the process of treating by syndrome differentiation. This article explained the formation and the connotation of the syndrome concept from the basis of the Chinese theory, treatment principle, and prescription and medication, according to the Chinese medicine syndrome.â‘ The concept of syndrome was formed by summarizing the symptoms and physical signs in accordance with the diseases, while the theory of TCM and traditional thoughts were implied. Qi-yinyang-wuxing theories,as the methodology, became an part of TCM theory when its connotation became distinctive from disease. State of viscera theory is the principal part of TCM that makes up of the theory mansion. Qi-yinyang-wuxing theory and state of viscera theory reflect the essence of TCM syndrome concepts. Therapy rule is confirmed by syndrome, which is also the representative of the essence of TCM syndrome concepts.â‘¡The treatment principle is based on the syndrome differentiation. The principle that regulates the functional imbalance to normal embodies the functional imbalance characteristics of the syndrome concept.â‘¢Prescription is based on the treatment methods, and the prescription is composed of Chinese herbs. Prescribing is the last step in the treatment of syndrome differentiation. By studying the herbs in the prescription,we can infer the syndrome of the disease. By analyzing the relation between the herbs and prescription, we made further study of the connotation of TCM syndrome concepts. Classical Shanghan Lun which reached the peak of perfection in the field of prescription and medication in TCM theory, concludes perfect prescription and precise therapy rule,discusses the relationship between disease, prescription and medication. The rules in the book are so definite that we can discover and grasp them easily. So I will research on classical prescription and medicament to hunt after the essence of TCM syndrome concepts reversely. And we can provide one way to the research of the essence of TCM syndrome concepts; on the other hand, we can expound the essence of prescription and medication.By the research, we believe that syndrome is a pathological summary which embodies the whole imbalance in the human body, and we can regard it as one kind of model. It has certain academic value and the practical significance in understanding the Chinese medicine syndromes concept, diagnosing and treating clinical diseases.
