Community medical service is a microcosmic composing part of city health service, andconstitutes national basic medical treatment service system with new countryside cooperationmedical service. So it is very necessary to analyse the function orientation and cost-income ofcommunity medical service.Now western welfare states energetically develop community medical service offeringmode in order to settle the sharp increasing problem of medical treatment service expenditureemerged in the 60's. The development of foreign community medical service has sixty toseventy years history, and psychopath, the elderly, pregnant and lying-in woman, children,and the handicapped are all serving objects. Serving contents aim at chronic-preventing, selfhealth care, healing and nursing work. General practitioner and dual transfer-examinationsystem have improved comparatively perfectly. We also set out to adopt this pattern in China.There are of course many benefits in China intrinsic big-hospital-centered medicaltreatment supply mode, but higher medical treatment cost and maximum waste of medicaltreatment resource are outstanding abuses of this mode. The developing extent and function ofnational community medical service have more difference and deficiency than westerncountries'. Therefore we need use the community medical service mode of developedcountries for reference to promote the improvement and development of Chinese PublicHealth enterprise. The article analyses its function orientation, benefits and defects by theangle of our traditional big-hospital-centered medical treatment service, and discussesdevelopment mode and the function-orientation benefits of our community medical serviceaiming to medical treatment resources wasting defects on account of higher medical treatmentcost.By the contrast and computation economics' demonstration analysis of different serviceitem's charge price between our big hospital and community hospital, and the cost-incomemethod analysis of community hospital, the article found community medical service hasuncomparable predominance on treating usual illness and excessive outbreaking diseases andhas feebleness competition power on treating difficulty and miscellaneous disease incompared with big hospital. If community hospital wants to develop more for treating difficulty and miscellaneous disease, it is difficult to keep cost-income balance and maintainsurvival and development with its existing manpower and material resources.As the closest health institution to people's life, Community medical service center is faroff lower than community hospital on charges because of cost factor, but it can't replacecommunity hospital on important status of medical service and is different from communityhospital. Accordingly the development direction is to form the mode of first examination incommunity medical service center, usual illness and excessive-outbreaking diseases withoperation treatment in community hospital, and difficulty and miscellaneous diseases'transfer-examination to big hospital or polyclinic.There are some relative literatures about cost-income analysis in theory on communitymedical service's supply mode and function in western countries. By the demonstrationanalysis of function orientation and cost-income between big hospital and community hospital,the article may give collocation efficiency research of public health resource in themathematic point of view. Meantime, it analyzes our community hospital developmentcondition, and will be help for the theory and practice of community medical treatmentsupply in our system in existence.
