Objective:Nasopharyngeal carcinoma(NPC)is characterized by a prominent lymphocytic infiltration.The expression of vascular cell adhesion molecule-1(VCAM-1)in unkeratinized nasopharyngeal carcinoma(UNPC)had been studied.This study was to observe the pattern of VCAM-1 expression in UNPC and the relationship between the expression of VCAM-1 protein and the T lymphocytes and some cytokines in unkeratinized nasopharyngeal carcinoma(UNPC).Methods:53 cases of UNPC and 23 cases of chronic nasopharyngitis(CNPi)were observed.In this study,we investigated any correlations between VCAM-1 and the count of T lymphocyte subtypes by the immuno histochemistry methods.Meanwhile the lymphocytes infiltrating in the UNPC tissue were observed on HE specimens. Furthermore,we examined the serum levels of soluble VCAM-1,IL-1β,IL-2,IL-4, IL-10 and the count of T lymphocyte subtypes in the peripheral blood of 34 patients with UNPC and 6 patients with CNPi by enzyme-linked immunosorbent assays(ELISA). All data were analyzed with UNPC patients' clinical status.Results:(1)Of the 53 UPNC tissues,49(92.5%)expressed the VCAM-1 positive signs on the tumor cells. The expression of VCAM-1 in the tissues of TNMⅠstage was much lower than that of TNMⅡ+Ⅲ+Ⅳstage(P=0.003).(2)Both UNPC and CNPi had a widely VCAM-1 expression on the epithelial cells.In the most of the CNPi tissues,vascular endothelial cells expressed VCAM-1 more or less.Contrastively,only a few of the blood vessel endothelial cell in the UNPC expressed VCAM-1.(3)The NPC patients had higher serum levels of soluble VCAM-1 than CNPi patients(P=0.012).The patients of TNMⅠ+Ⅱstage had higher serum soluble VCAM-1 levels than those patients with TNMⅢ+Ⅳstage(P=0.028).And the serum soluble VCAM-1 levels in the UNPC patients with T1+2 stage were higher than those with T3+4 stage(P=0.009).(4)The expression of VCAM-1 in the UNPC tissues was significantly negative with the serum soluble VCAM-1 level(r=-0.353,P=0.040).(5)There were no significant correlations between the expression of VCAM-1 and the count of T lymphocyte subtypes,the same as the degree of lymphocytes infiltrating in and between the carcinoma nests.There were no significant correlations between soluble VCAM-1 and the count of T lymphocyte subtypes in the peripheral blood,however the higher serum level of sVCAM-1 was significantly associated with IL-1βand IL-4 in UNPC patients with TNMⅠstage (r=0.769,0.745;P=0.037,0.045).Conclusions:(1)The VCAM-1 protein was expressed by tumor cells of most UNPC.(2)The elevated sVCAM-1 was an early event while the VCAM-1 expressed in UNPC tissue gradually strong accompany with UNPC staging.(3)IL-1βand IL-4 take part in the procedure of inducing sVCAM-1 increase at the early stage of UNPC.(4)In UNPC,the VCAM-1did not show the effect to recruited the T lymphocytes to the tumor tissue.(5)UNPC patients' immunity significant decreased characterized by decreased counts of T lymphocytes in early stage,and might mainly by inhibited T lymphocytes' function in next stages.
