Abstract/Summary:
salpingectomy and ovarian cystectomy were ordinary surgeries in the field of gynecology. Many infertile patients prepared to receive in vitro fertilization-embryo transfer (IVF-ET) who had undergone the two kinds of surgery. Because the blood vessel and nerves of ovary and oviduct are in close vicinity, salpingectomy may destrory ovarian function. Ovarian cystectomy may destrory normal ovarian tissue so that it may affect ovarian function. Many investigator pay close attention to the problem at the present time, but their conclusions are not identical. In order to investigate the effect of salpingectomy and ovarian cystectomy on ovarian function, the research compare the ovarian volume , the antral follicle count(AFC), and ovarian stromal blood flow indices of infertile patients underwent unilateral salpingectomy or ovarian cystectomy. We choose fifty-three infertile women in our hospital. Thirty-two patients underwent unilateral salpingectomy and twenty-one patients underwent unilateral ovarian cystectomy. We analyze thirty-one IVF-ET treatment cycles in thirty-one infertile patients who previously had undergone unilateral salpingectomy or ovarian cystectomy. According to the number of the number of developed follicles class the poor-response group and the normal-response group. Compare the ovarian volume, AFC and the ovarian stromal blood flow Resistance index (RI),Systolic/diastolic (S/D) and Pulsatility index (PI) between the operated side and non-operated side of the two kind of the operated groups and between the poor-response group and the normal-response group. There were no difference between the operated side and non-operated side underwent unilateral salpingectomy or unilateral ovarian cystectomy in the ovarian stromal blood flow indices RI, S/D and PI(P>0.05). The antral follicle count (AFC) and ovarian volume in the operated side underwent unilateral ovarian cystectomy were significantly lower than the non-operated side (P<0.05). There were no difference between the operated side underwent unilateral salpingectomy with the non-operated side in the items of AFC, ovarian volume. The antral follicle count (AFC) was significantly lower in the poor-response group compared with the normal-response group (p<0.05). The ovarian volume, RI, S/D and PI were not significantly different in the two groups (p>0.05).Ovarian blood flow was not changed after salpingectomy or ovarian cystectomy. Transvaginal color Doppler measurement of the ovarian stromal blood flow indices have no predictive value of ovarian response. The results showed that ovarian volume and AFC was not impaired after salpingectomy and the ovarian volume and AFC seems to be impaired after ovarian cystectomy.
