Acute ischemia cerebral vascular disease(AICVD) is a common disease in the old people, and has the characterristics of high morbidty,high relapse and high incidence of causing disability. Forthermore, it may threaten people's lives. TCM has showed the great superiority in curing AICVD. Therefore, to study the entiopathogenesis and the pathology, physiology of AICVD, to find out proper, efficent and convenient TCM treatment is one of the purpose of the clinical and experimental study of TCM.This paper includes two parts, summarization and experimental study. The summarrization summarrizes the frame of reference of TCM to AICVD, summarries the studies that how acupuncture or medicine to influent the contents of NO in blood serum or ischaemic brain tissues in recent years and the status that acupuncture how to treat AICVD. The expermient is as following:Objective: To observe the morphologic changes and inflammatory reaction of rat model of acute focal cerebral ischemia at 12h,24h,3d , the changes of the contents of TNF-αand NO in the blood serum , and the influence of application of combining acupuncture with herbal medicine. The purpose is to find better therapy through comparing curative effect of combined therapy with single acupuncture therapy, and the regulating mechanism of combined therapy, and offering a new effective therapy to clinic treatment.Methods: SD rats(male, weight 320±20g, 150)were randomly distributed to control group(15), model group(45), acupuncture group(A group,45), combined therapy group(C group,45); Occlude middle cerebral artery of rat by suture method for creating the model of acute focal cerebral ischemia and reperfusion; the rats of A group and C group were punctured Baihui point passing through Qubing point at planning time. After being punctured the rats of C group were clysised Naoxuekang into stomach. Observe neuron ischaemic necrosis of brain tissue, inflammatory immune response changes, inflammatory cell adhesion and infiltration by hematoxylin-eosin staining(HE); detect NO contents in blood serum through the means of deoxidizing nitric acid enzyme; detect TNF-αcontents in blood serum through the means of with filling double antibodies.Results: (1) In the early stage of acute focal cerebral ischemia and reperfusion (12h-3d), degeneration and necrosis of neuron, accumulation, adhesion and infiltration of inflammatory cell occurred. The scope and extent of brain tissue necrosis expanded and deteriorated with progressing of disease and continuing of inflammatory reaction. Combined therapy is able to reduce the inflammatory cell adhesions and infiltrations, and alleviate inflammatory immunoreactions and neuron necrosis. (2) NO contents in blood serum are higher than regular at 12h,24h,3d after acute focal cerebral ischemia(P<0.05), and the trend is higher and higher. Combined therapy or acupuncture therapy both can debase the NO contents in blood serum(P<0.05), and combined therapy is better than acupuncture therapy at 24h,3d(P<0.05). (3) NO contents in blood serum are higher than regular at 12h,24h,3d after acute focal cerebral ischemia(P<0.05), the NO contents in blood serum at 12h is highest in the three time point, it is lower and lower from 24h to 3d, Combined therapy can debase the TNF-αcontents in blood serum(P<0.05), and combined therapy is better than acupuncture therapy (P<0.05).Conclusion: Combined therapy can restrain the produce of some free radical and reduce some inflammatory cell cytokines contents, as well as releases of cell toxic substances, thereby oppressing the delayed neuronal damages, ameliorating the microcirculation, and promoting brain blood supply, contributing to the recovery of neural function. Moreover the regulation of combined therapy is better than single acupuncture therapy.To sum up, combined therapy can regulate the system of Hypothalamus- Hypophysis-Suprarenal gland, and not only influent nervous- incretion- immunity regulation meshwork, but also regulate effectively balance of meshwork of some cells cytokines. so the disorder functions can be modulated to protect the brain. All that may be one of the most important mechanisms of combining acupuncture with herbal medicine to treat ischemic cerebral vascular diseases.
