| Essential hypertension (EH) is a kind of common disease in clinic,which can lead to some severe complications such as cerebral stroke,cardiovascular disease,renal damage,etc.The prevalence rate of this disease presents an ascendant tendency with the rapid development of economic and the growing tendency of aged society.Differentiation of syndromes is the basis in treating EH in Traditional Chinese Medicine (TCM).The Objective of this study is to discuss the distribution regularity of TCM syndrome patterns of EH and the correlation between syndrome patterns and some objective examinations.It is expected to provide some evidences of differentiation of syndromes and to improve the level of differentiation of syndromes and treatment in EH.Methods:200 patients of EH classified into four patterns as liver-fire hyperactivity,yin deficiency and yang hyperactivity, excessive phlegm-dampness,yin-yang deficiency are investigated.The data of patients such as height,weight,blood pressure(BP),blood lipid,blood sugar are collected and analyzed by SPSS13.0 statistical analysis software packet.Results:1. Liver-fire hyperactivity takes up 22.5%,yin deficiency and yang hyperactivity takes up 36.5%,excessive phlegm-dampness takes up 28.5%,yin-yang deficiency takes up 12.5%.There is a significant statistical difference in the sex over the distribution of syndrome patterns.2.The distribution of syndrome patterns of grades and risk stratifications of EH is also different.3.There is a significant statistical difference in BP and the course of EH among patients with different syndrome patterns.4.The incidence of complications is also different.5.Syndrome patterns of TCM of EH have something to do with some factors including sex,age,high intake of salt,alcohol,tensity,impatience,low physical activity,BMI,LDL,E/A,EF,etc.Conclusion: 1.Liver-fire hyperactivity,yin deficiency and yang hyperactivity and excessive phlegm-dampness are primary syndrome patterns of EH.Excessive phlegm-dampness is the most common pattern in male patient,and yin deficiency and yang hyperactivity is the most common one in female patient.2.Liver-fire hyperactivity,excessive phlegm-dampness and yin deficiency and yang hyperactivity are major patterns in grade 1 of EH.Yin deficiency and yang hyperactivity, excessive phlegm-dampness and liver-fire hyperactivity are major patterns in grade 2 of EH.Yin deficiency and yang hyperactivity, excessive phlegm-dampness and yin-yang deficiency are major patterns in grade 3 of EH. Liver-fire hyperactivity and excessive phlegm-dampness are major patterns in low risk group and medium risk group of EH. Yin deficiency and yang hyperactivity and excessive phlegm-dampness are major patterns in high risk group of EH.Yin deficiency and yang hyperactivity and yin-yang deficiency are major patterns in very high risk group of EH.3.The growth of diastolic pressure(DBP) is significant in the patterns of both liver-fire hyperactivity and excessive phlegm-dampness.The increases of systolic pressure(SBP) and DBP are significant in the pattern of yin deficiency and yang hyperactivity. The increases of SBP and pulse pressure(PP) are significant in the pattern of yin-yang deficiency.4.Liver-fire hyperactivity and excessive phlegm-dampness are major patterns in middle-aged patients whose course of EH is short.The pattern of yin-yang deficiency expands and the pattern of liver-fire hyperactivity decreases with the increase of age and course.Yin-yang deficiency is major pattern in old patients whose course of EH is long.5.There are various complications in the pattern of yin-yang deficiency,while there are less complications in the pattern of liver-fire hyperactivity.Coronary heart disease and heart failure are often found in the patterns of both yin-yang deficiency and excessive phlegm- dampness.Diabetes is often found in the patterns of both yin-yang deficiency and yin deficiency and yang hyperactivity. Hyperlipemia is often found in the pattern of excessive phlegm-dampness. 6.The patient will probably be differentiated as the pattern of liver-fire hyperactivity when he is tense,impatient and in normal diastolic function.If the patient is female,55~74 years old,high intake of salt and normal in LDL,she is probably differentiated as the pattern of yin deficiency and yang hyperactivity.The patient will be probably differentiated as the pattern of excessive phlegm-dampness with low physical activity,alcohol abuse,obesity,high LDL and below 55 years old.If the patient is above 75 years old and descendant in EF, he is probably the pattern of yin-yang deficiency. |
