ObjectiveThe prevalence of nonalcoholic fatty liver disease(NAFLD) hasincreased with years, whereas its pathogenesis still remain unclear upto now with the lack of an effective drug in clinical treatment.Professor's self-made Cailoutiaozhi Decoction based on the principlesof invigorating spleen, regulating liver, promoting blood circulation andresolving phlegm has better therapeutic effect on NAFLD than others.To validate the mechanism and effect of Cailoutiaozhi Decoction inclinic, and finally to provide the evidence of theory and experiment, weobserved the mechanism and effect of Cailoutiaozhi Decoction to treatrats'NAFLD induced by high fat diet.Methods1 Grouping Method: 61 male SD rats were randomized into normal group(n=6), model group(n=11), Cailoutiaozhi Decoction treatment groupswith lower, medium and high dosages separately(n=10), UrsodeoxycholicAcid treatment group(n=10).2 Modeling Method: The normal group were fed with common feed, whilethe other 5 groups were fed with high fat feed for 12 weeks.3 Administration: From the beginning of the modeling, each rat inmodel group was gaster-poured by deionized water with 10ml/kg,the ratin Cailoutiaozhi Decoction treatment groups was gaster-poured by lower,medium and high dosages of Cailoutiaozhi Decoction with 10ml/kg(containing processed samples 2.2g/ml, 1.1g/ml, 0.55g/ml separately),the rat in Ursodeoxycholic Acid treatment group was gaster-poured byUrsodeoxycholic Acid with 10ml/kg(concentration of 9.45mg/ml). Each ratwas gaster-poured once a day for 12 weeks.4 Target examination: The rats were executed at the 12th week of themodeling,each weight was mensurated, the blood samples were collectedto detect the activities of the serum ALT, AST and the TC, TG Content,each liver was weighed and the wax sections of liver tissues were HEstained,The classifications of hepatic fatty degenerations andinflammations were evaluated under the light microscope. ResultsNo difference for weight was found in groups.Liver Index(wet weight/body weight×100%): model group's(4.68±0.24)% was remarkably increased(P<0.01) comparing with normalgroup's(2.87±0.18)%, Cailoutiaozhi Decoction treatment groups'was significantly lower than model group's(P<0.01), UrsodeoxycholicAcid treatment group's(4.55±0.38)% had no significant differencewith only a slight decreasing tendency.Histopathological Analysis: In model group, hepatic steatosis withmainly macro-lipid-droplets happened and assosiated with monocytes'infiltration into inter-lobular stroma, its classifications of hepaticfatty degenerations and inflammations was remarkably higher thannormal group's(P<0.01). Cailoutiaozhi Decoction treatment groups'classifications of hepatic fatty degenerations and inflammations wereremarkably higher than model group's(P<0.01,P<0.05). CailoutiaozhiDecoction treatment groups with medium and high dosages'classifications of hepatic fatty degenerations and inflammations wereboth higher than control groups'(P<0.05).The serum ALT, AST: The activities of serum ALT,AST in model groupincreased highly compared with normal group(P<0.01). The activitiesof serum ALT,AST in Cailoutiaozhi Decoction treatment groups with highdosages decreased significantly compared with model group(P<0.05),whose level of ALT was similar to normal group(P>0.05). The levelsof serum ALT,AST in Ursodeoxycholic Acid treatment group had nosignificant difference with only a slight decreasing tendency(P>0.05).The serum TG, TC: The level of serum TG in model group increasedhighly compared with normal group(P<0.05). The level of serum TGin other groups decreased a little compared with model group(P>0.05).The level of serum TC in model group increased a lot compared with normalgroup(P<0.01). The level of serum TC in Cailoutiaozhi Decoctiontreatment groups with medium and high dosages decreased a lot comparedwith model group(P<0.01), but was significantly different with whichin normal group(P<0.01). The level of serum TC in UrsodeoxycholicAcid treatment group had no significant difference compared with bothCailoutiaozhi Decoction treatment groups and model group(P>0.05). ConclusionsCailoutiaozhi Decoction based on the principles of invigoratingspleen, regulating liver, promoting blood circulation and resolvingphlegm has better therapeutic effect on NAFLD than others by improvinghepatic inflammation and injury, reducing fat deposition, finallyprotecting the liver. Its mechanism has probable relations to theregulation of the serum TC, TG's metabolism and Anti-lipidperoxidation.
