| Objective To summarize the cytogenetic characteristics and their clinicalsignificances of the patients with multiple myeloma(MM) in China..Methods Cytogenetic findings of 100 patients with MM were retrospectivelyanalysed, and the cilincal implications of specific chromosomal abnormalities (CA)were investigated. Result1. Cytogenetic findings : clonal CAs were detected in 16 of 93 patients (17.2%) byconventional cytogenetics ( CC ); in the numerical aberrations, the most commontrisomies were 11,21, 3,6 and 12, and the most common monosomies were 16, 13, 8, 22 and 11;in the structural aberrations,the most frequently involved arms were 14q, 8q, 1q,6q, 11q, 12p and 1p, and the most frequently involved breakpoints were 14q32> 8q22~24,11q13~14, 6p21,1q10~11 and 12pl2~ 13: ploidy levels in decreasing order were hypodiploidy (37.5 %), pseudodiploidy(31.3%), hyperdiploidy (18.8%) ,tri-/tetraploidy (12.5%) ;chromosome 13abnormalities (C13As) were detected in 7/100 patients(7%) ,and its prevalence in CC and interphase fluorescence in situ hybridization (FISH) were 6% and 33.3% respectively;translocations involving chromosome 14 and/or 14q32 aberrations were detected in 6 patients(6%);2. Clinical implications:there was no significant correlation between C13As, hypodiploidy, hyperdiploidy and the known conventional prognostic factors ;on univariate analysis, C13As, hypodiploidy and clonal CA were associated with significantly shorter overall survival (OS) and time to progression (TTP);on multivariate analysis,C13As was the only independent unfavorable factor—the median OS, TTP of patients with or without C13As were 6 months versus 50 months (P=0.005) , 0 months versus 35 months (P=0.016) ,respectively;the Intergroup Francophone du Myelome (IFM) staging system ,which incorporates C13As,proved to be more effective in prognostic differentiation than International... |
