Points Set To Take Acupuncture Method Standardization And The Establishment Of A Control Group To Control The Quality Of Impact

1 BackgroundThe curative effect of acupuncture depends on acupoint. stimulation methods and people's condition,therefore locating acupoint,practicing acupuncture methods normatively and setting up scientific control group are the sticking points which affected the quality of acupuncture researches.Nowsdays most studies about acupuncture have problems including: (1) Not considering the individual difference in the course of locating acupoint and actualizing acupuncturejacking standardization scheme;(2) Not establishing the logical control group,it isn't according to the objective completely,on the other hand,the using of placebo acupuncture control group is little.And all of these influenced the quality of acupuncture researches and exerting its function.2 ObjectiveInvestigating the acupoint locating , acupuncture methods standardization and setting up the logical and scientific control group.Evaluating the control effect of routine acupuncture,shallow acupuncture and placebo acupuncture.3 Way of thinkingOn the basis of systematic review of acupuncture RCT literature,and taking touci therapy treating periarthritis of shoulder as the example we develop the clinical research from three methods: normalize locating tiaokou point,standardization touci therapy and establish different control groups.4 Contents and Results4.1Systematic review about acupuncture RCT literaturesWe choose literatures published in two kinds of possessing representative magazines which are 《Acupuncture of China》 and 《Combanation of TCM and western medicine of China》 from 1981 to 2002.We search all the acupuncture RCT researches,adding up to 460,and evaluating them according to the below six standards: (1) whether the objective is clear-cut;(2) whether it have the choosing criteria(including diagnosis criteria,including and excluding criteria);(3 )whether considering the individual difference in locating acupoint and practicing the acupuncture therapy;(4) whether describing the details of acupuncture therapy clearly,including locating acupoint ,the angle,depth,intensity,direction of acupuncture;(5) the specific control method;(6) whether having the according of control group.The result of systematic review cue:the capital agents affecting quality of acupuncture research including: (1 )patients,including the disease and its clinical situation;( 2 )acupuncture theraphyjncluding the behavior of patients and doctors,locating acupoint and the specific theraphy;(3) whether the control group is reasonable and whether it's according to the objective. 4.2 Research of standardization of acupoint locating and touci therapy practicing 4.2.1 Standardization of acupoint locatingWe chose 6 volunteers,first they would be femiliar with the method that we made aboutlocating tiaokou acupoint and the normal way ,then they use these two methods to locate tiaokou acupoint,we measured the distance from the acupoint that they located to external malleolus and the distance from the acupoint to anterior crest of the tibia ,and then we got two data groups,we statisticed and analysised the data we got through two different ways to verdict whether there was difference between the two groups.The results diplay:We use the non-paramete Kendall's check to analysis the data,there is no differece among the data from the standardization way,and there are marked difference among the data from normal way. 4.2.2 Standardization of touci theraphyTaking tiaokou acupoint stab deeply to chengshan acupoint as an example, we discussed the depth about the touci therapy by two ways: (1) review articles: taking touci therapy as the key words,and search the articles in CMCC database,the literatures adding up to 109,and the depth of touci therapy is according to the different part and disease,and 8 of them adopt 75mm needle and the depth is from 2 cun to 2.5 cun. (2 ) consulting the experts, by having an informal discussion and filling in the questionnaire we definite the schem of touci therapy. 4.3 Evaluation about control effect of different control groupThis department including: (1) Definiting whether the shallow acupuncture and placebo acupuncture can be blind for patinents;(2) Taking touci therapy for periarthritis of shoulder as the example,proceed clinical research about different control group for 120 patients4.3.1 Research about the effectiveness of minimal acupcnture and placebo acupunctureWe chose 10 volunteers, and on their tiaokou acupoint we put to use the touci therapy ,normal deep needle^ninimal acupuncture and placebo needle to measure the VAS scale to find whether the placebo control acupuncture can be blind to the patients. We found that there was no difference between the placebo control acupuncture and the routine needling methods,so we can conclude that needling sensation between placebo control acupuncture and routine needling methods was no differece^ninimal acupuncture and placebo needle can be used in clinical research as the placebo acupucnture control group.4.3.2 Clinical research about establishing different control groupWe choose tiaokou acupoint to treat periarthritis of shoulder,and touci therapy as the treatment group,the normal deep needle^ninimal acupuncture and placebo needle as the control groups.Through statistic about the curative effect about touci and normal deep needle methods,we wanted to judge whether touci therapy was more effective than normal deep needle ,on the other hand through analysised the cural effctiveness of minimal acupuncture and placebo control acupuncture we wanted to judge which placebo effect was more sensitive. By time march^006,we implement 120 qualified cases,touci group 31,normal group 29,minimal acupuncture 29 and placebo acupuncture 31.We use SPSS 11.0 software to analysisi data/esult as follow:Through treating periarthritis of shoulder by tiaokou acupoint,we can get two conclusions, first normal deep needle as the control group could be the effective control group to prove the different curative effect ,and touci therapy was more effective than normal deepneedling, second there was no difference between minimal acupuncture and placebo needle about their placebo effect,so we need extend the sample size and make further study about it 5 Conclusion(1) The factors that affected the quality of acupuncture research,especially the control quality are:patients,acupuncture therapy,the reasonable of control group;(2) tiaokou acupoint to treat periarthritis of shoulder,establishing normal acupuncture as the control group can exclude the placebo effect and also prove the superiority effect oftouci;(3) Minimal acupuncture and placebo needle can be used in clinical research,but there are no difference about the placebo effect5.1 Locating acupoint is one of the key factors that affected the quality of acupuncture control group.
