Abstract XiaoChaiHu Decoction (Minor Bupleurum Decoction) consisted of Bupleurum, Scutellariae, Pinellia, Date, Ginseng, liquorice, ginger. It is one of the most famous decoctions of the Treatise on Febrile and Miscellaneous Diseases. It is used to strengthen the body resistance, remove pathogenic factors and reestablish normal correlation between the interior and exterior of the body, for the treatment of syndrome of Shaoyang Meridian, attack of heat on the blood chamber(uterus) in women, jaundice and malaria. At the same time, it can also clear heat and regulate the balance of Heart, Lung, Liver, Gallbladder, and resolve phlegm. This famous decoction is one of the most common used decoction in Japan. Its indication is abroad, including the disease of liver and gallbladder, the disease of respiratory system, externally contracted disease, psychopathy, and neuropathy, among which virus hepatitis is most frequently encountered diseased. But Among the chronic hepatitis patients treated with XiaoChaiHu Decoction, some appears interstitial pneumonia, which is considered as one of the side-effect of XiaoChaiHu Decoction. But in China, there is no interstitial pneumonia report from the patients treated with XiaoChaiHu Decoction. This paper review the current usage status of XiaoChaiHu Decoction in Japan, and explore the factors contributing to the interstitial pneumonia induced by XiaoChaiHu Decoction on the chronic hepatitis patients. Conclusion:1. 1970's XiaoChaiHu Decoction was popular in Japan as the treatment for the chronic hepatitis, and had good clinical effect. The mechanism of XiaoChaiHu Decoction was deeply studied. XiaoChaiHu Decoction has the multi-effect on the body, such as regulating immune, anti-inflammatory and protecting the injury of hepatic cells. But the mechanisms of XiaoChaiHu Decoction for treating chronic virus hepatitis is the same mechanisms for treating interstitial pneumonia and pulmonary cirrhosis.2. After the interstitial pneumonia induced by XiaoChaiHu Decoction was reported in 1989, the side-effects reports of XiaoChaiHu Decoction increased. Summary these reports, we think the possibility of inducing interstitial pneumonia by long time usage of XiaoChaiHu Decoction on chronic hepatitis patients.3. From the aspect of the symptoms differentiation of TCM, which is not in Japan,interstitial pneumonia belong to the Yin deficiency.The Yin deficiency which is induced by XiaoChaiHu Decoction according to the Japanese doctor is the result of mistreatment by Japanese doctor.
