Objective To investigate the associatoin of C-C chemokine receptor 5 (CCR5) gene promoter region 59029G/A polymorphism with diabetic nephropathy in kunming Han Chinese. Methods The polymorphism of CCR5 gene promoter 59029G/A in 163 unrelated Kunming Han type 2 diabetes (including 53 normal albuminuria DNO group, 64 microalbuminuria DN1 group and 46 macroalbuminuria DN2 group) and 60 normal control were detected by polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP).The genotype frequency and allele frequency and relative clinical data were compared between groups. Results (1) The age Waist hip ratio in DN2 were higher than DNO and the systolic blood pressure diabetic duration the ratio of patients with hypertension were significantly higher than DNO and DN1. (2) The G/A A/A genotype frequencies and A allele frequencies in DN groups (DN1+DN2) were significantly higher than DNO . (3) There were no significant differences in genotype frequencies or allele frequencies between DN1 and DN2. (4) Although there were statistic differences in genotype frequencies, there were no statistic differences in allele frequencies between DNO and NC. (5)There were no significant difference in clinical data in type 2 diabetes with different genotype. (6) Logistic regression analysis showed that CCR5 gene promoter region 59029G/A polymorphism and diabetic duration were significantly related to DN. Multinomial logistic analysis showed that CCR5 gene 59029A-positive genotypes were risk factors of DN development and the diabetic duration of over than 5 year were risk factor of DN2. Conclusion The CCR5 promoter region 59029A-positive genotypes may be risk factors of DN development; When diabetic duration is over than 5 years, the patients with microalbuminuria may be prone to macroalbuminuria stage in Kunming Han Chinese.
