Purpose:To investigate the operative techniques and evaluate the outcome of autogeneic hamstring tendons with preservation of the remnants and the remainder of the fibre single-stranded reconstruction of anterior cruciate ligament (ACL)and discuss some relevant issues.Material and method:From Auguest 2008 to July 2010 ,68 cases (45 men and 23 women, whose age is between18 to 45and the average age is 28.2 )of ACL rupture were treated with single—bundle ACL reconstruction , autogeneic hamstring tendons with preservation of the remnants and the remainder of the fibre single-stranded reconstruction of anterior cruciate ligament The tibia remnant and the synovial shell were preserved as much as Possible.Grafts were pulled intra-artieularly through the tibia1 remnant under arth- roseopic visualization. The ACL tibia1 remnant was compacted by the tendon passage. A11 patients were followed up for more than one year as scheduled.femur use mini-plate and titanium button to built , Anterior drawer test (ADT),Lysholm Tegner Aand,International Knee Documentation Committee(IKDC)rating scale were used to evaluate the curative effects after the operation.Results:All of them were followed up for 12 to 25 months with an average of 16 months Anterior drawer test (ADT)was negative in all cases .Lachman test was negative in 66 cases,and positive in 2 case.All cases in the pivot shift test(-) the International Knee, Documentation Committee (IKDC) TOP SCOREs that normal cases were 65(95.6%),near normal cases were 3(4.4%) .the International Knee, Documentation Committee (IKDC) criteria, subjective knee score was 94.6±3.2at the last follow-up.Lysholm knee function score was remarkablely improved between (95.8±2.8) scores .Conclusion : ACL Reconstruction with remnants and remaining bundle Preservation is beneficial for the early revascularization and fibroblasts'growing of thegraft , and improves tendon healing inside a bone tunnel.Arthroscopic reconstruction of ACL with the remaining bundle preserving technique could facilitate revasculization and reinnervation of the reconstructed ligament and maintain stability of knee joint.
