Objective:To investigate the clinical value of 256-detector spiral computed tomography(CT) in assessing global left ventricular function compared with magnetic resonance imaging (MRI) as the reference standard. Methods:Twenty-three patients(10 males,13 females,63±4 years) with suspected coronary artery disease(CAD) underwent coronary angiography using 256-detector spiral computed tomography. A further cardiac MRI examination was performed within one week period. With semiautomatic contour detection software (Cardiac Viewer software, EBW workstation), the parameters of end diastolic volume(EDV),end systolic volume(ESV),stroke volume(SV),ejection fraction(EF) and cardiac output (CO) were determined from CT reformation images created at every 5% of R-R interval (0-95%). The results from CT were compared with those from MRI using paired t test,Pearson correlation and Bland-Altman analysis. Results:No significant differences were found using paired t test among the parameters of global left ventricular function obtained with 256-detector spiral CT and MRI besides SV. EDV, SV, and EF were slightly overestimated by 256-detector spiral CT compared with MRI, and ESV was slightly underestimated by 256-detector spiral CT compared with MRI. However, the parameters of global left ventricular function obtained with 256-detector spiral CT were significantly correlated with the MRI data(r=0.855-0.934, P<0.01). A strong correlation between CT and MRI was also found in the Bland-Altman analysis. The Bland-Altman analysis also showed that the repeatability of 256-detector spiral CT was well. Conclusion:256-detector spiral CT and MRI showed a high level of correlation and agreement in assessing the left ventricular function parameters.256-detector spiral CT can accurately and reliably assess the left ventricular function. It is a valuable method for evaluating cardiac function.
