[Objective] To estimate the Burden of diabetes mellitus in China by epidemiologic and economic, To give overview of the influence of diabetes mellitus to the health of Chinese people.[Method] According to the population data from CHINA STATISTICAL YEARBOOK (2009) compiled by National Bureau of Statistics of China, the epidemical data from The fourth health service survey of china in 2008 compiled by the Minister of Health and other survey in literatures, and the cost data from the CHINA HEALTH STATISTICAL YEARBOOK (2009) compiled by the Minister of Health and The fourth health service survey of china in 2008, this thesis calculated the burden of diabetes mellitus in 2008. The methodology of DALYs created by The WHO was introduced in this paper to calculate the epidemic burden of diabetes. Step-model method was introduced to calculate the direct economic burden of diabetes, and human-capital method combining DALY was used to calculate the indirect economic burden of diabetes.[Result] In china, the burden of disease of diabetes mellitus in 2008 was 9151134 DALYs in all, including 4598900 DLAYs contributed by male,4552234 DALYs contributed by female. The DALYs contains YLLs (Years of Life Lost due to mortality) and YLDs (Years Lost due to Disability).There are 1826056 YLLs,844809 for male,981247 for female, and 7225079 YLDs,3754091 for male,3470988 for female.Eliminated population factors influence, the burden of diabetes mellitus in 2008 was 6.83 DALYs per 1 000 people,6.74 DALYs per 1000 people for male,6.93 DALYs per 1000 people for female. There are 1.24YLLs per 1000 people for male,1.53 YLLs per 1000 people for female; and 5.50 YLDs per 1000 people for male,5.40 YLDs per 1000 people for male. The direct burden of diabetes mellitus was estimated by the combination of the average expenses per inpatient and the number of inpatients, and the average expenses per outpatient and the number of visits. The direct burden of diabetes mellitus in 2008 was 67.75 billion including 63.09 billion of the direct medical expenses, and 4.66 billion of the direct expense not for medical. Because of the economic, many patients can't afford to go to hospital; we also estimated the potential expense 14.51 billion. The indirect burden of diabetes mellitus is 75.36 billion, which contribute to an important part of the total burden of diabetes mellitus, higher than the direct burden of diabetes mellitus for 7.51 billion.[Conclusion] The results in this study show that DALYs for females exceeded that for males; in the total population, the DALYs of the population at 45-60 years old were more than the others. Therefore, we suggest that it is the focus of screening and prevention for diabetes mellitus above 45 or the younger high-risk population for reducing the burden of disease.Finally in order to more accurate computation burden of the disease to provide health policy recommendations and the international comparisons, we suggested that we should enhance the monitoring of chronic diseases, especially for the burden of the disease, to provides perfect basic data.
