Objective: To filter serum proteomic signs of hepatic echinococcosis, and establish the model of serum protein fingerprint patterns for diagnosis of hepatic echinococcosis, and provide a foundation for study of molecular mechanism of Echinococcosis. Methods: Serum samples from 68 patients with hepatic echinococcosis confirmed by surgery and pathology, 33 patients with other liver disease and 40 normal humans were analyzed using SELDI-TOF-MS and CM10 ProteinChip. ZJU-Protein Chip Data Analyze System was used to analyze the different protein peak spectrum expressed in serum of patient group and control group and establish a model for diagnosis of hepatic echinococcosis. Blind methods was used to evaluate the value of the model. Results: There are nine different protein peak spectrum between patients group and normal group, of which one protein peak spectrum increased and eight protein peak spectrum decreased in patients group (P<0.05). The model based on ten protein peaks whose P values are the smallest could do the best in diagnosis of patients and controls. It's sensitivity and specificity validated by blind method was 90.3% and 70.0%; Two different proteins were detected from patients with hepatic echinococcosis and other liver disease, and both increased in other liver disease(P<0.05). The sensitivity and specificity of this model was 87.1% and 56.2%; There are nine different protein peak spectrum between patients group and control group combined by normal group and other liver disease group, of which eight protein peak spectrum whose relative molecular mass are 1044, 1047, 1073, 1075, 1338, 6453, 6649, 8714 m/z respectively decreased in patients group, while the 5651 m/z protein peak spectrum increased (P<0.05), The sensitivity, specificity, positive predictive value and negative predictive value of the diagnostic model validated by blind method was 77.4%, 66.7%, 66.7%, 77.4% respectively; Two different proteins were detected from patients with hepatic cystic echinococcosis and hepatic alveolar echinococcosis; Six different protein peak spectrum were detected from patients with hepatic echinococcosis before surgery and after surgery. Conclusion: It is successful to find serum specific signs in patients with hepatic echinococcosis by SELDI-TOF-MS and Bio-informatics and establish a diagnostic model, which have broad clinical value in judging improve and researching pathology mechanism.
