Objective:To approach the character of 1H Magnetic resonance spectroscopy (1H MRS) and Diffusion-weighted imaging(DWI) of the focal Encephalitis related Human acquired immunodeficiency syndrome.To search the qualitative diacritic index of 1H MRS and DWI for AIDS related focal encephalitis and discriminating AIDS related focal encephalitis and HIV negative focal encephalitis. Methods:Performed conventional MRI,Magnevist Solution /Magnevist contrast enhancement,1H MRS,DWI in 14 AIDS related focal encephalitis and 14 HIV negative focal encephalitis(controls). 62 1H MRS, the data were acquired and areas of the six peaks of NAA,Cr,Cho,LL(Lac and Lip),mI,Cho+ mI measured in the parenchymatous tissue,zone of necrosis and edema focus of infection,the metabolite ratios of NAA/Cr,Cho/Cr,LL/Cr,mI/Cr,NAA/Cho,LL/Cho,mI/Cho,NAA/mI,(Cho+mI)/NAA were calculated.57 DWI, the signal and apparent diffusion coefficient(ADC) were measured. To compare 1H MRS and DWI of the parenchymatous tissue,zone of necrosis and edema focus of AIDS related focal encephalitis, and controls.Results:A reduction in NAA and increase in Cho,LL and mI in the parenchymatous tissue; a reduction in NAA,Cho and increase in LL,mI in the zone of necrosis and edema;Cr no change. Lap was observed in all part of AIDS related focal encephalitis, 22.85%in the zone of necrosis,17.14%in the parenchymatous tissue,14.29%in the edema; Lip was observed 5.71% in zone of necrosis.A statistically significant(p<0.05)difference ratios of LL/Cho,Cho/Cr,(Cho+mI)/NAA in all part of AIDS related focal encephalitis. To compare controls, A statistically significant(p<0.05) difference ratios of LL/Cr,LL/Cho,mI/Cho in the parenchymatous tissue and LL/Cr,LL/Cho in the zone of necrosis,the chang of AIDS related focal encephalitis was small than controls. DWI was mostly hypersingal(85.71%). A statistically significant(p<0.05) difference ADC in all part. To compare controls, A statistically significant(p<0.05) difference ADC in the parenchymatous tissue,the change of AIDS related focal encephalitis was large than controls.Conclusion:There were a significant character of 1H MRS and DWI of AIDS related focal encephalitis.The index for diagnostic and distinguish were discovered.
