Background and Objective:Gastric carcinoma (GC) is one of the most common primary malignant tumors, and its morbidity and mortality are all high. In china, the morbidity of gastric carcinoma is the first in digestive tumors. 160,000 patients die from gastric cancer each year, and it threatens the health and life of mankind seriously. For a malignant tumor, its occurrence , development, infiltration and metastasis is the procedure of several phases, multiple gene participation and several transcription factors regulating, and refer to a series of patho-physiologic reactions, including cell abnormal reduplication, adhereing, motion, ECM degraduation ,angiogenesis and so on.The leading causes of the mortality are infiltration and metastasis of gastric cancer. So exploring the mechanism of the invasion and metastasis of gastric carcinoma has become one of research hot spots. It is very important of the expression of Ets-1 and MMP-9 during GC occurs, grows, invases and transfers angiogenesis and so on. Up to date, domestic and foreign documents have seldom reports on researches in expression of Ets-1, MMP-9 and Detection of MVD in gastric carcinoma and their relationship mutully.In order to discuss the mechanisms of infiltration and metastasis of gastric carcinoma and search valid methods which can suppress the infiltration and metastasis of gastric carcinoma, immunohistochemistry SP method is adopted to observe the expression of Ets-1 and MMP-9, and detect MVD in 70 cases of gastric carcinoma tissues, 40 cases of adjacent non-atypical hyperplasia tissues and 28 cases of normal mucosa tissues... Detecting unitedly illuminates furtherly the mechanisms of infiltration and metastasis of gastric carcinoma and provides theory foundation for blocking infiltration and metastasis of gastric carcinoma.Methods1. Using the immunohistochemistry SP technology to detect the expression of Ets-1 and MMP-9 and MVD in the70 cases of gastric carcinoma, 40 cases of tumor-adjacent atypical hyperplasia tissues and 28cases of normal mucosa of gastric carcinoma respectively.2. Statistical analysis: SPSS 10.0 for windows were used Statistic analysis. The Chi-square was used in the comparison of positive rates; the t-test was used in the comparison of means; the relation of two variables was analyzed by the spearman level correlation analysis. The level of significant difference was a=0.05.Results1. The positive expression rates of Ets-1 protein in normal mucosa of stomach , atypical hyperplasia tissues,and carcinoma tissues were 46.4%(13/28), 57.5%(23/40) and 87.1% (61/70) respectively; the positive expression rate of Ets-1 protein in the carcinoma tissues was compared with that in the propotional adjacent tissues and normal mucosa tissues from each other, there was statistical significance(P<0.05).2. The positive expression rates of MMP-9 protein in normal mucosa of stomach , atypical hyperplasia tissues, and carcinoma tissues were 39.3%(11/28),60%(24/40) and70% (49/70) respectively; the positive expression rate of MMP-9 protein in the carcinoma tissues was compared with that in the propotional adjacent tissues and normal mucosa tissues from each other, there was statistical significance(P<0.05).3. The means of MVD in normal mucosa of stomach, atypical hyperplasia tissues and carcinoma tissues were 11.29±1.08,12.98±1.62 and 30.16±3.92 respectively, the mean of MVD in the carcinoma tissues was compared with that in the propotional adjacent tissues and normal mucosa tissues from each other, there was statistical significance(P<0.05). 4. In the 48cases of lymphnode metastatic group, the positive rate of expression of Ets-1 protein in the tumor tissues was 95.8%(46/48); in the non-lymphnode metastatic group, it was 68.20% (15/22) . There was significant difference between the two groups (P<0.05).5. In the 48cases of metastatic group, the positive rate of expression of MMP-9 protein in the tumor tissues was 83.3%(40/48); in the non-metastatic group, it was 40.9% (9/22) . There was significant difference between the two groups (P<0.05).6. In the tissues of gastric carcinoma, the positive rates of expression of Ets-1protein on the gastric mucosa layer invaded, muscle invaded and outer layer invaded were 58.30% (7/12),88.90% (24/27) and 96.8% (30/31) respectively. The expression of Ets-1 protein in the carcinoma tissues with the mucosa layer invaded was evidently lower than that in the tissue with muscle invaded and outer layer invaded. There was significant difference among the groups (P<0.05). Thus the expression of Ets-1 protein in the carcinoma tissues with muscle invaded was compared with that with outer layer invaded, there was no significance (P>0.05).7. In the tissues of gastric carcinoma, the positive rates of expression of MMP-9 protein on mucosa invaded, muscle invaded and outer layer invaded were 25.0%(3/12),66.7% (18/27) and 90.3% (28/31) respectively. The expression of MMP-9 in 3 kinds of carcinoma tissues grew up gradually. There was significant difference among the groups (P<0.05).8. In the tissues of gastric carcinoma, The means of MVD on mucosa invaded, muscle invaded and outer layer invaded were 25.79±2.95,29.92±2.93,32.05±3.66 respectively. Their mean in the carcinoma tissues increase gradually. There was significant difference among the groups (P<0.05).Conclusions1. The positive rates of the expression of Ets-1 and MMP-9 protein in normal mucosa tissues, tumor-adjacent atypical hyperplasia tissues and gastric carcinoma tissues of stomach increased up gradually, and the positive rates of them in case of early stage GC was higher than that in late stage GC. It indicated that the two proteins participated in occurrence and growth of gastric carcinoma. The positive rates of the expression of Ets-1 and MMP-9 protein in gastric carcinoma tissues increased gradually accompanied with the invasion depth, the expression of them in gastric carcinoma tissues with lymphnode metastasis was higher than that in non-lymphnode metastasis.2. In GC, expression of Ets-1 and MMP-9 proteins are unconcerned with patient sex and age . Expression of Ets-1 protein is also unconcerned with degree of tumor cell's differentiation, yet expression of MMP-9 protein related with it.3. The expression of Ets-1 was positively correlated with that of MMP-9, the means of MVD in case of Ets-1 and MMP-9 protein positively were higher than that negtively. It suggested that they play an important role in GC's infiltration, metastasis and angiogenesis. Perhaps they can be one of important early diagnosis and prognostic indexes in GC.
