Objectives:To observe the expression of IGF-1 in the peri-hematomal brain region of Intracerebral Hemorrhage(ICH) patients,study the role of IGF-1 in secondary cerebral injury, and investigate into the relationship among Excess Syndrome of Stroke and Prostration Syndrome,the expression of IGF-1,volume of blood tumor,scores of NIHSS and GCS.To provide the molecular biology standard for the microcosmic syndrome differentiation with TCM to quest for a new treatment.Materials and methods:28 patients with ICH recollected and divided into three groups:YIN excess group,YANG excess group and Prostration group.All were accord with the diagnosing points of ICH and must be able to be diagnosed as one of the syndromes based on the differentiation standards of TCM and undergone surgical operation,among which 14 were male and 14 were female.The age was(61.18±8.86) years,the course of disease was 6(4-9) h(nonnormal distribution).YANG excess patients was 13,among which 7 were male and 6 were female.The age was(62.31±10.71) years,the course of disease was(6.85±3.41) h.YIN excess patients was 8,among which 4 were male and 4 were female.The age was(60.00±7.45 )years,the course of disease was(6.75±3.92) h.Prostration Patients was 7,among which 3 were male and 4 were female.The age was(60.43±7.32) years,the course of disease was (7.00±3.83) h.6 normal brain tissue were taken as normal control group.We collected the perihematomal brain tissue in the course of evacuation of hematoma in ICH group.The NIHSS and GCS were used to score,the DUOTIAN formula was used to calculate the hemorrhage volume,the IGF-1 expression was detected with Western blotting method.Results:1.The expression of IGF-1 in the peri-hematomal brain region of ICH patients significantly increased in comparison with the normal control groups(P<0.01).2.The expression of IGF-1 was significantly different among YIN Excess Syndrome,YANG Excess Syndrome and the prostration Syndrome(P<0.01,P<0.05),The expression of IGF-1 of prostration Syndrome was the highest,YANG Excess Syndrome second,YIN Excess Syndrome last.3.The hematoma volume and The scores of NIHSS and GCS were significant different among YIN Excess Syndrome,YANG Excess Syndrome and the prostration Syndrome in ICH(P<0.01).The volume and the scores of prostration Syndrome were the first.While YANG Excess Syndrome second and YIN Excess Syndrome were the last.The last one was opposite.4.There were positive correlation among the expression of IGF-1 and scores of NIHSS (P<0.01).There were negative correlation among the expression of IGF-1 and scores of GCS (P<0.01).The expression of IGF-1 showed obvious positive correlation with the hematoma volume(P<0.01)Conclusion:1.The expression rate of IGF-1 in the peri-hematomal brain region of ICH patients significantly increased in comparison with the normal control groups(P<0.01).The expression of IGF-1 was positive correlation with the tumor volume and NIHSS score,was negative correlation with GCS score.Suggest that IGF-1 participated in the secondary cerebral injury of cerebral hemorrhage.IGF-1 may be an important inhibition of apoptosis.It can improve neuronglogical deficit after ICH.The higher the expression of IGF-1,the more severe of the disease.2.The IGF-1 level maybe provide the molecular biology standard for the microcosmic syndrome differentiation with TCM.Combining the expression of IGF-1 in brain tissue with the volume of bleeding,GCS and NIHSS score maybe provide the molecular biology standard for the microcosmic syndrome differentiation with TCM.3.Both the Excess Syndrome of stroke and prostration Syndrome in ICH could reflect the degree of brain impairment.4.The study showed:IGF-1 participated in the secondary injury of ICH,the function which is antiapoptosis.If IGF-1 is given to the ICH patients,It is impossible to improve the patients prognosis obviously.It can provide the new direction for the cerebral hemorrhage's treatment.
