PurposeThe purpose of this study is to understand the financial burden of the disease status of patients with lung cancer, analysis of medical security system to alleviate the financial burden of patients with lung disease, the role, for the rational allocation of limited health administrative departments of health resources, control and reduce the economic burden of disease, provide the scientific basis for further improvement of the medical security system for reference.Data sources and methodsSource: Based on the principle of random sampling, in Cancer Hospital information systems of Hubei, community information on lung cancer patients were randomly selected and, in accordance with the extent of 100 people selected as the household survey sample. Methods include: (1) literature: the economic burden of disease, cancer, cost, medical expenses, the cost of medical insurance for the key words, by domestic and foreign periodical literature net to collect information on the research methods were analyzed. (2) on-site investigation: including questionnaires and unstructured interviews. Patients with lung cancer questionnaire, mainly socio-economic conditions for the lung cancer patients, the first six months after discharge from hospital (including the first hospital) reimbursement for out-patient and hospitalization fees, transportation, escort fees, payments, purchase their own drug costs and the main care of the socio-economic status of caregivers of patients spent time and money as well as various indirect burden. Non-structural interview: The interview about the family got sick with lung cancer before and after the quality standard of living changes, understand their current level of fees, health insurance policies and reimbursement levels of view, listen to their suggestions. (3) statistical methods: descriptive studies, and regression analysis. Descriptive statistical study of the socio-economic, demographic and health service utilization. Regression analysis was used to observe the direct economic burden of disease factors.Result1. The burden of patients with lung disease. The direct economic burden per capita reached 57,638.26 yuan, per capita health care of the direct economic burden of 41,293.94 yuan, is half the family income were 228.96%, to 9911.52 yuan per capita indirect costs, accounting for 55.76% of household income patients. Poverty caused by a very high rate, nearly 100%.2. Lung cancer patients directly affect the economic burden of disease factors were age, occupation, whether there is health insurance.3. The situation of different groups with different economic burden. From the age point of view, the greater the economic burden of age, the more light; from the employment point of view, jobless and unemployed patients than in the industry, the financial burden of patients with higher retired; from income levels, economic burden as a percentage of revenue income increase be lowered; different medical insurance cost burden on different functions, the highest proportion of publicly funded health care reimbursement, medical insurance for urban residents, and a new rural cooperative medical compensation is relatively low, at the same time, compensation levels and the cost burden is not proportional to the percentage of revenue.4. Medical insurance direct economic burden on patients to relieve significant role. Reimbursement through medical insurance, per capita direct medical economic burden by 46.54%. Reimbursement rate of 60.04 percent outpatient, inpatient reimbursement rate of 55.35%, significantly reduced the proportion of patients pay the direct cost of treatment. But the pay is still a heavy burden.Proposal1. To improve health insurance coverage. Vigorously promote medical insurance for urban residents. Increase the proportion of health insurance compensation, improve medical insurance compensation programs to reduce poverty caused by the rate.2. To enhance the patient's social support, focusing on the patient's hospice.3. To improve the level and form of medical assistance. Civil affairs departments should be effective in patients with severe economic burden of poverty alleviation mission.4. To strengthen publicity and education of lung cancer, raise public awareness of prevention, healthy lifestyles, the implementation of tobacco control legislation.
