Objective:The diabetes mellitus is one of key factors for the attacks of cerebrovascular diseases, the status of illness for the patients of diabetes mellitus is grave, its prognosis is bad, and also has a high mortality rate. We have given a study on the 217 patients who are suffered from cerebral infarction. We made the comparison from the aspects such as clinic biochemistry, grading defective nerve function and iconography. Taking the methods of MIR and MRA, we made the quantitative study on the quantity and pathologic change position of intracranial vascular. Also made the comparison with the patients of NDACI. This paper will discuss the diagnose value of MRI and MRA that used in the pathologic change degree for the DACI patients. Make out the pathologic change for cerebrovascular disease between DACI and NDACI patients, find out their quantity differences and analyze the reason to provide important reference for the early diagnose and proper treatment. of DACI patients.Methods:During the period of September 2006 and September 2007, we have chosen the patients in the Affiliate Hospital of North China Coal Medical College who are cerebral infarction as the target, the selection criterions of the case are as follows: first, the initial attack of disease, second, the duration of the attack is less than 48 hours. The grouping is as follows: according to the diabetes mellitus record and after check the blood sugar iterating and diagnosed as the diabetes mellitus: the group of DACI about 107 patients, the NDACI 110 patients ( the diagnose of diabetes mellitus type two and diagnose criterion for cerebral infarction refer to the content of the paper). The patients in these two groups are certified that there is no hemorrhagic cerebrovascular disease and cerebral tumor in the examination of CT. The clinical symptoms has not alleviated for 24 hours, scan the skull by using the methods of MRI and MRA after 24 hours CT and ahead of 72 hours. Observe the number of infract focal and its position, the type and amount of pathological change of vascular, and then made the analysis and comparison, all the patients should be asked their medical history in details and be carefully examined, the DM patients ask questions on the treatment, the status of blood sugar control, and the possibility of DM complication. Make the assessment on all the patients about their conditions and the defective status nerve function, take the medical treatment for four weeks towards these three groups of people according to cerebral infraction and diabetes mellitus standards. After that make assessment on laboratory of haemopoiesis which includes the blood sugar, blood-fat, blood stream change, fibrinogen, saccharogenic hemoglobin etc. cardiogram, color ultrasonic check on carotid, and fill in the questionnaire, and then make the statistic analysis of the data and draw the conclusion.Results:1. The level of TC,LDL,FIB,HBA1,FBG,PBG in DACI group is higher than that from NDACI,there are obvious statistic sigificant ( P<0.01). The two groups exist significant differences in the gravity of defective nerve function ( P<0.01).2.The group of DACI increases more significantly than those from NDACI group in the aspects of lung infection, urinary infection or else like retina pathological change, peripheral nerve pathological change, and DACI group is much higher in the hemiplegia, pathology positive ( P<0.01) compared with NDACI, the NDACI group has a higher recovery of illness, but the DACI group are much higher in the change or deterioration of the disease, the death rate is high( P<0.05), there are significant difference in the dementia and melancholia between these two groups of patients.3.There are no obvious differences in the infracted position and comparison amount between the two groups. The results showed in the MRI are alike, The DACI group in the field of the lacumar cerebral infarction has great statistic significance compared with the NDACI group(P<0.05),The proportion is higher in focal disposition distributed around the area of blood-supply (P<0.01) . the pathological change of vein is dominated by the vascular obstruction and hemadostenosis and also has the statistics significance compared with NDACI group(P<0.05).Conclusions:1.The diabetes mellitus is an independent and controllable critical factor in the cerebral infarction, the state of illness is relative heavy, it develops fast, the rehabilitation is slow.2.The incorporate cerebral infarction with diabetes mellitus are common seen in the multiple and small infarction focal. The infracted area is the artery in the cerabra blood–supply part, the basal node, corona radiate semi-egg circle center are more liable to weary.3.Along with the development course of diabetes mellitus, the number of infracted focal and pathological changes of vascular increased gradually, and the pathological changes in the blood vessel commonly involved in more than two vessels and with the characteristic of narrowness and obstruction.
