Objective To research the value of sputum culture on identify the pathogens of community-acquired-pneumonia, and the guidance meaning of sputum culture on helping treating community-acquired-pneumonia,, and to compare the diagnosises of Mycoplasma pneumoniae between fluorescent PCR and ELISA , and to observe the effects of moxifloxacin empirical sequential therapy of community-acquired-pneumonia.Methods 100 patients of mild to moderate community-acquired pneumonia and 100 patients of acute exacerbation chronic obstructive pulmonary disease were enrolled, culture the sputum sample timely, the differences between the results of the sputum culture were observed; 60 patients with Mycoplasma-pneumoniae-infection and 60 patients with non-Mycoplasma-pneumoniae-infection were selected, both were detected Mycoplasma pneumoniae with fluorescent PCR and ELISA, the discrepancy of the two methods were observed; 196 patients of mild to moderate community-acquired pneumonia were randonmly divided into the experimental group (injection: Moxifloxacin 0.4 ivgtt qd×5d + tablet: Moxifloxacin 0.4 po qd×5d ) and control group(injection: Ceftriaxone 1.0 ivgtt qd×10d, injection: Azithromycin 0.5 ivgtt qd×5d+ tablet: Azithromycin 0.5 po qd×5d ),all the patients were anti-infective therapied 10 days, and observed the clinical efficacy.Results The positive rate of Community-acquired pneumonia sputum culture was 12%,while the positive rate of AECOPD was 67%(P<0.05);The sensitivity and specificity of fluorescent PCR was 93.3% and 90% respectively. The sensitivity and specificity of ELESA was 91.7% and 65% respectively. The clinical efficacy of experimental group was 96%, while the control group was 94.8% (P>0.05).Conclusions The guidance meaning of sputum culture for treating mild to moderate community-acquired pneumonia is limited, The clinical efficacy that Moxifloxacin treat empirically community-acquired pneumonia is good, and have few adverse reactions.
