Objective:To compare the alignment of computer-assisted TKA with the conventional technique.And find the problem of present navigation system, propose the resolve means, guide clinical treatment.Method:The object of this study is middle and old aged arthritic who need to accept primary surface TKA.Two groups of 120 TKAs each were operated using either a CT-free navigation system or the classical surgeon-controlled technique. The postoperative leg alignment was analysed on long-leg coronal X-rays. Incremental deviations to the left or right were summed and compared according to the following categories:no deviation(0°),a deviation of 1°to 3°and a deviation of>4°from the 0 axis.Pearson's chi-squared test and SPSS statistics software were used for testing significance. The aim is to discuss the contribution of navigation system in the primary surface TKA. Result:In navigated group,deviation of 0°/1°~3°/>4°in mechanic axis was achieved in 69(57.5%),51(42.5%),0(0.0%)respectively,while in hand -free group, deviation of 0°/1°~3°/>4°in mechanic axis was achieved in65(54.2%)/65(54.2 %))/16(1.3%)respectively.In navigated group,deviation of 0°/1°~3°/>4°in femoral front axis was achieved in 82(68.3%),37(30.8%),1(0.8%)verus hand-free group 63(52.5%)/23(19.2%)/34(28.3%)respectively.,Deviation of 0°/1°~3°/>4°in mechanic axis In navigated group was98(81.7%)/21(17.5%) /1(0.8%)verus85(70.8%)/23(19.2%)/12(10.0%)respectively.The result revealed a highly significant of navigation with regard to the mechanical axis(chi-squared test =26.8,P<0.0001),the frontal femoral axis(chi-squared test=38.3,P<0.0001)and the frontal tibial axis(chi-squared test=14.53,P<0.05).Conclusion:The use of navigation system was shown to improve the alignment of the implant.
