Objective:To study on the safety,feasibility and invasion of laparoscopic hepatectomy (LH).Methods:To comply with a standard 45 patients with hepatectomy were divided into LH group and open hepatectomy(OH) group randomly in the first affiliated Hospital of Shihezi university from April 2008 to Nov.2009. Compared with patients of two groups including length of incision,operation time,intraoperative blood loss, postoperative eating date, dosage of analgesic use, postoperative hospital stay, complications, body temperature, hemogram, humoral immunity (IgG, IgM, IgA, IgE) and cellular immunity during the perioperative period.Results:①The mean of data such as the length of incision (4.27±1.01cm), blood loss (75.00±57.05ml),dosage of analgesic use (11.00±11.36mg) and hospital stay (6.27±1.84d) in LH group was significant postoperatively less than that of OH group. There were no significant difference of surgical duration between the tow groups,②The mean of eating date in LH group (1.55±0.68d) and in OH group(3.00±0.36d) was significant difference(P<0.05),there were no complication in LH group,2 patients happened bile leak,1 patient happened ascites,③there were no significant difference of temperature in preoperation between tuo groups④The mean of data of WBC and NE wre no significant difference in preoperation between tuo groups,but which of LH was lower than OH.⑤cellular immunity between two groups were no significant difference (P>0.05).Conclusion:Under the circumstances of properly selecting patients,Compared with OH,LH is a safety and minimal invasion method. associated with minimal surgical trauma,less blood and fast postoperative recovery if we selected patients properly.
