Objective:To investigate the prevalence, clinical and immunological features of type 1 diabetic patients in the First Changde hospital.Subjects and Methods:According to diagnostic criteria of type 1 diabetes from WHO and IDS, we screened type 1 diabetic patients from hospitalized diabetic patients in the First Changde hospital from 2005 to 2009. And we collected the clinical data and detected GADA and IA-2A with radioligand assay (RLA). Then we investigated prevalence, clinical and immunological features.Results:1.Type 1 diabetes accounted for 3.0%in the consecutive hospitalized diabetes. Classical type 1 diabetes, latent autoimmune and idiopathic type 1 diabetes accounted for 85.2%,3.5%and 11.3% respectively in total type 1 diabetes.2.The number of rural type 1 diabetic patients attendance trended to increase year by year (P>0.05), and the ratio of DKA in rural patients was higher than that in urban patients(P< 0.05).3.In adult type 1 diabetic patients with less BMI(BMK24kg/m2), PBS and FCP were higher (P<0.05).5.Compared with patients in IA-2A single positive group, patients in GADA single positive group were at older age(p<0.05). patients in GADA single positive, GADA and IA-2A double positive and double negative groups had a lower level of FBS and PBS (P<0.05).6. The frequencies of GADA and/or IA-2A were higher than single antibody in type 1 diabetes (P=0.094, P<0.05).Conclusions:1.Type 1 diabetes accounted for 3.0%among the consecutive hospitalized diabetic patients in the First Changde Hospital.2. The number of rural type 1 diabetic patients attendance trended to increase year by year, and the ratio of DKA in urban patients was higher. 3.1A-2A positive rate was higher in children onset type 1 diabetic patients than that in adult onset type 1 diabetic patients.4. Diagnostic efficacy of classical type 1 diabetes could be enhanced by combined determination of GADA and IA-2A.
