Study On Traditional Chinese Medicine "xushi" Dialectic Treatment For Primary Abdominal Distension In Critical Patients

Objective:TO observe the clinical effect of TCM"xushi" dialectic treatment for early abdominal distension in critical patients with decoction of "Houpu sanwu" and decoction of "Houpu shengjiang banxia gancao renshen",which derived from Zhang Zhongjing's "Shanghanzabinglun", compared with western medicine Domperidone Tablets.Methods:120 critical patients of early abdominal distension, according to TCM"xushi" dialectic theory, were randomly divided into four groups. "Xuzheng" test group (30 cases) and control group(30 cases); "shizheng"test group (30 cases) and control group(30 cases). "Xuzheng"test group were treated with decoction of HP-SJ-BX-GC-RS and "shizheng"test group with that of HPSW; control group with western medicine Domperidone Tablets.48 hours was a course of treatment.. The changes of every group in APACHEⅡ,TCM symptom scores, Gastric intromucosal pH(pHi), the intra-abdominal pressure, cytokines were observed before and after treatment. Results:Both the test group and the control group lower significantly APACHEⅡ, TCM symptom scores, the intra-abdominal pressure and increase gastric pHi, but the test group is obviously superior to the control group (P<0.01). In addition, the test group can significantly reduce cytokines such as TNF-a, IL-6, but there was no difference in the control group (P> 0.05). Gastric pHi of test group is normal after treatment,while that of the control group is abnormal still..Effective rate is 90% and 73.3% respectively in abdominal distension "shizheng", the test group is significantly higher than controls (P<0.01). Effective rate is 86.7% and 66.7% respectively in abdominal distension "xuzheng", the test group is also significantly higher than controls (P<0.01). Conclusions: Clinical effect of TCM"xushi"dialectic treatment for early abdominal distension in critical patients with decoction of HPSW and that of HP-SJ-BX-GC-RS is much better than Western medicine.And they can remove inflammatory mediators and improve gastrointestinal blood perfusion.
