Objective To investigate the effects and mechanism of XIAO YAO SHU KUN TANG which taking functions of dispersing the depressed liver-energy, regulating vital energy, dissolving stasis and alleviating pain as the principle of composing prescription to treat chronic pelvic inflammatory disease of qi-stagnancy and blood stasis type. Methods 60 patients suffering from chronic pelvic inflammatory disease of qi-stagnancy and blood stasis type were divided two groups at random.30 cases in the treatment group were treated with XIAO YAO SHU KUN TANG, The other 30 cases in the control group were treated with FU YAN KANG FU tablet. The changes of the symptoms, body signs were observed. The concentrations of the T cell subsets (CD3,CD4,CD8), the natural killer cells (NK cell) and the tumor necrosis factor (TNF) in peripheral blood were determined respectively, the changes of indexes of hemorheology (whole high blood viscosity, whole low blood viscosity, plasma viscosity, RBC hematocrit, ESR, aggregation index, fibrinogen) and the changes of the size of the mass and the indexes of the hemodynamics of the uterine artery were determined. Results The disorder of immune function, high blood hyperviscosity and stagnant blood, microcirculation disturbance of pelvis were found in the cases of chronic pelvic inflammatory disease of qi-stagnancy and blood stasis type. After treatment the total effective rate of the treatment group and control group was 90.00% and 63.33% respectively, the marked effectiveness rate of the treatment group and control group was 70% and 33.33% respectively, There were significant differences (P<0.05). The effect of the treatment group were better than the control group in changing the clinical symptoms and body signs (P<0.05), regulating the immune function (concentration of,CD4,CD8, ratio of CD4/CD8, concentration of TNF and NK, P<0.05) and promoting the microcirculation of the pelvic (P<0.05). The improvement of the index of hemorheology in the two groups were significant after the treatment (P<0.05). After the treatment, the improvements of indexes of hemodynamics of uterine artery in two groups were obvious compared with the condition before treatment (P<0.05). There were significant differences in the comparison of the two groups after treatment. Conclusions XIAO YAO SHU KUN TANG had the functions of regulating the immune function, modifying the microcirculation of pelvic, amending the index of hemorheology, ameliorating hemodynamics of uterine artery and improving the clinical manifestations and body signs. Regulating the immune function may be one part mechanism of treating chronic pelvic inflammatory disease.
