With the development of computer network and modern communications technology, telemedicine is gradually moving from concept to applications and from theoretical study to practical research and development of system. Telemedicine is a new work mode of medical treatment and medical education. It is the combination of modern medicine, computer science technology and telecommunications technology. This work mode breaches both the limitation of medical resources distributing and the limitation of time and space. It can achieve a high degree of sharing of medical resources, and produce a positive effect to effective nurturing and accumulation of new medical resources.At present, telemedicine systems that based on special high performance networks are already very powerful and advanced. These telemedicine systems usually have a high performance of networks, and can provide plenty excellent complicated services. However, because of its small scope, and costly, it is difficult to universal apply as public telemedicine network. In order to enlarge the development space of telemedicine system, we must find a widely used, more powerful network, and so far Internet is the only choice.Internet-based community medical consultation and assistant diagnosis are important components of telemedicine system, and it is also a hot issue of telemedicine system development. As people's living standard has being improved and life style has being changed, the number of elderly and people with chronic diseases in the community is growing. Implementation of an effective health care service can greatly improve their quality of life, and bring a positive significance to the harmonious development of society. Patients can obtain equivalent medical service indoors with what is available in hospital by building a community telemedicine consultant system, and this will balance the medical resources distribution in different areas.In order to satisfy the requirements of community medical clinic and modern family health care, this paper designs a set of SIP-based community telemedicine diagnosis system software based on SIP, P2P and multimedia network programming. The design of this software is on the basis of the key technology and implementation issues study of telemedicine diagnosis system which is suitable for community, it combines the technology project item of Shandong Education Office "the Research and Development of Virtual Hospital Integration Platform" and enterprise commissioned subject "Research and Development of××community medical diagnosis system in Jinan City ". The main work of this paper is as follows:(1)First of all, the status of modern telemedicine consultant system research is analyzed, and the advantages and disadvantages of current popular solutions are summarized. In order to solve a series of problems such as heavy burden on center server, mobility of consultant doctors, diversity of patients, a model of community telemedicine consultant system is developed based on P2P and SIP, and the key technology, software and hardware structure, running processes of system are discussed.The whole system is comprised of remote portable medical terminal, remote consultant video assistant, electronic medical records module and database server module.(2) Considering the demand of the project, on the basis of study for the community telemedicine consultant system based on SIP, software terminal are developed using the multimedia network programming and development of oSIP protocol stack.The terminal design is based on P2P, which features two endpoints of communication as equivalent, real time multimedia data intercommunicates without server, reducing the burden of server as a result. Terminal software is developed in environment of Visual C++ 6.0, it accomplishes the SIP function via oSIP protocol stack, and finishes the multimedia communication function based on Microsoft Real-Time Communications (RTC) API. The terminal software performs well at the lab with small net delay of video calls, which can meet the demand of remote consultant system.(3) At last, a summary of this paper is proposed and future improvements on the basis of this paper are suggested.
