Study On The Immune Mechanism Of Yongquan Acupoint Attention Therapy In Patients With Idiopathic Pulmonary Fibrosis

Objective:Idiopathic pulmonary fibrosis susceptible(Yang-deficiency constitution) population are research objects, and research on immunological mechanism in the treatment of idiopathic pulmonary fibrosis(IPF) with cataplasm of application on Yongquan during three nine-day period based on combined formula of “collateral disease theory”.Materials and Methods:The study based on preliminary questionaires of pulmonary diseases susceptible constitution showed that susceptible constitution of IPF was Yang-deficiency constitution,and it also indicated that application on Yongquan point therapy could improve clinical symptoms,blood gas analysis and 6-minute walk test for Yang-deficiency constitution IPF.According to constitution classification method of Constitution of Traditional Chinese Medicine by Wang qi, 30 patients with idiopathic pulmonary fibrosis who met inclusive criteria of Yang-deficiency constitution were selected,and before and after self control design schema was used, cataplasm of application on Yongquan therapy was given during three nine-day period,and the course as 30 days.Safety indicators and cytokine IL-8,IL-12,IL-13,TGF-β1,TNF-α, IFN-γ level in serum and T lymphocyte subpopulation CD3+,CD4+,CD8+level in the peripheral blood and concentration changes of serum immunoglobulin Ig G,Ig M,Ig A were detected before and after 3 days into the erollment. SPSS17.0 statistics software was performed to analyze the study results,then research on immunological mechanism with application on Yongquan point therapy in the treatment of idiopathic pulmonary fibrosis.Results: 1.Comparison of serum IL-8,IL-12,IL-13 level for patients with Yang-deficiency constitution IPF before and after application: Serum IL-8 level was lower than that before application,and the difference was statistically significant(p <0.01); Serum IL-12 level was no significant changes compared to that before application and the difference was no statistically significant(p >0.05); Serum IL-13 level was also lower than that before application,and the difference was statistically significant(p <0.01). 2.Comparison of serum TGF-β1,IFN-r,TNF-αlevel for patients with Yang-deficiency constitution IPF before and after application: Serum TGF-β1 level was lower than that before application,and the difference was statistically significant(p <0.01); Serum IFN-r level increased compared to that before application and the difference was statistically significant(p <0.01); Serum TNF-αlevel was also lower than that before application,and the difference was statistically significant(p <0.01). Lavender 2016/05/18 10:26:07 3.Comparison of T lymphocyte subpopulation CD3+,CD4+,CD8+level in the peripheral blood for patients with Yang-deficiency constitution IPF before and after application: CD3+ level in the peripheral blood was lower than that before application,and the difference was statistically significant(p <0.01); CD4+ level in the peripheral blood increased compared to that before application and the difference was statistically significant(p <0.01); CD8+ level in the peripheral blood was lower than that before application,and the difference was statistically significant(p <0.01). 4. Comparison of serum Ig G,Ig M,Ig A level for patients with Yang-deficiency constitution IPF before and after application: Serum Ig G level was no significant changes compared to that before application,and the difference was no statistically significant(p >0.05); Serum Ig M level was no significant changes compared to that before application,and the difference was no statistically significant(p >0.05); Serum Ig A level was also lower than that before application,and the difference was statistically significant(p <0.05).Conclusion: Treatment for Yang-deficiency constitution IPF with application on Yongquan Point is related possibly to its regulating immuno-inflamatory reaction,regulating Th1/Th2 cytokines disproporation and correcting CD4+/CD8+ disproportion:in some degree decreasing IL-8、IL-13 level in serum,inhibiting the generation of inflammatory factors,reducing various active substance caused by leukocytes chemotaxis, reducing the injuries of extrapulmpnary cellular matrix,meanwhile correcting Th1/Th2 cytokines disproportion; decreasing serum TGF-β1, TNF-α level and improving IFN-γ level, thereby reducing TIMP expression and fighting against lung collagen deposition,alleviating inflammation;reducing CD3+,CD8+ level in peripheral blood for patients with pulmonary fibrosis,and improving CD4+ level,and correcting CD4+/CD8+ disproportion,and regulating normal phase immunity.It should be further studies whether application has the biphasic regulation effects on serum immumoglobulin Ig G,Ig M,Ig A for patients with IPF.
