Retrospective Analysis Of 108 Cases Of Irritable Bowel Syndrome (diarrhea Type)

Purpose:Through a retrospective study,to investigate the clinical characteristics and TCM syndrome distribution of the patients with IBS-D from The Affiliated Hospital of Liaoning University of TCM in the past 5 years.The research analysis the TCM drugs and combinations of drugs commonly used,and then summarize the regularity of IBS-D treatment,in order to provide an objective basis for the future conduct targeted research,and provide guidance and assistance for clinical treatment.Material and method:Retrospective study the age,gender,occupation, predisposing factors,duration,onset season,TCM syndrome type and the characteristics of TCM drugs of108 cases of IBS-D patients in digestive ward and Outpatient department from January 2010 to September 2015.SPSS17.0 statistical software was used in the analysis of related indicators as mean, standard deviation,constitute ratio,then use chi-square test,unified hypothesis testing using two-sided test,P<0.05 is considered statistically significant.In the part of data mining,the association rules used for analysing the relationship between traditional Chinese medicine was based on GRI algorithm in Clementine12.0 data mining software,to summarize the core combination of commonly used traditional Chinese medicines.Results:1.General data analysis:The study included 108 cases,including 50 cases of male(46.3%)and female 58cases(53.7%),the sex ratio was 1:1.16,the average age was(51.54±14.99)years old.In this study,the highest proportion was middle-aged patients with 43.5%(P<0.05);In the occupation distribution,the largest proportion were the staff and the retiree with 56.5%( P<0.05);The main cause of IBS-D were the emotional factors and dietary factors with59.2%(P<0.05);In the study,the shortest duration was 6 months, the longest was 40 years,the largest proportion was 1-5 years with 43.5%(P<0.05);The disease occurrence all the year round,and mainly in summer with 41.7%(P<0.05).2.TCM Syndrome Distribution:TCM type divided into four types,stagnation and spleen deficiency syndrome 46cases(42.6%),weakness of the spleen and stomach syndrome 39cases(36.1%),spleen kidney yang deficiency syndrome 8cases(7.4%),spleen stomach damp heat syndrome 15cases(13.9%),stagnation and spleen deficiency syndrome and weakness of the spleen and stomach syndrome made up the largest proportion(P<0.05);Different TCM syndrome distribution in all age groups was statistically significant(P<0.05),middle-aged patients were mainly with stagnation and spleen deficiency syndrome and weakness of thespleen and stomach syndrome; There was no significant difference in the distribution of TCM syndrome types of different genders(P>0.05).3.TCM drugs statistics:By statistics,the total number of drugs was 114,the total frequency of drugs was 1344 times,the top 20 drugs were:Poria,Coicis Semen,Atractylodis Macrocephalae Rhizoma,Glycyrrhizae Radix Et Rhizoma,Dioscoreae Rhizoma,Amomi Fructus,Nelumbinis Semen,Citri Reticulatae Pericarpium,Astragali Radix,Lablab Semen Album,Bupleuri Radix,Paeoniae Radox Alba,Ostreae Concha,Platycodonis Radix,Saposhnikoviae Radix,Amomi Fructus Rotundus,Coptidis Rhizoma,Bletillae Rhizoma,Codonopsis Radix,shenqu.The high--frequency drugs of different syndromes:Top10 drugs of stagnation and spleen deficiency syndrome:Atractylodis Macrocephalae Rhizoma,Poria,Citri Reticulatae Pericarpium,Coicis Semen,Bupleuri Radix,Glycyrrhizae Radix Et Rhizoma,Dioscoreae Rhizoma,Amomi Fructus,Paeoniae Radox Alba,Nelumbinis Semen,and they were the main ingredients of An important formula for treating painful diarrhea and Bupleurum powder for soothing the liver;Top10drugs of weakness of the spleen and stomach syndrome:Dioscoreae Rhizoma,Poria,Coicis Semen,Amomi Fructus,Atractylodis stomach damp heat syndrome:Coicis Semen,Poria,Astragali Radix,Dioscoreae Rhizoma,Bletillae Rhizoma,Coptidis Rhizoma,Pogostemonis Herba, Glycyrrhizae Radix Et Rhizoma,Artemisiae Scopariae Herba,Puerariae Lobatae Radix,Scutellariae Radix,and they contained the major pharmaceutical composition of Pueraria,Scutellaria and Coptis Decoction.4.Data mining association rules analysis the compatibility of traditional Chinese medicines:the combination of two drugs commonly used:Nelumbinis Semen and Coicis Semen,Nelumbinis Semen and Dioscoreae Rhizoma,Nelumbinis Semen and Amomi Fructus,Lablab Semen Album and Glycyrrhizae Radix Et Rhizoma,Lablab Semen Album and Dioscoreae Rhizoma,Nelumbinis Semen and Poria,they were main in compatibility.Commonly used drug combination mainly reflected the effectiveness of strengthening the spleen and replenishing qi and promoting the circulation of qi and damp to stop diarrhea.Conclusion:1.General data analysis:The incidence of IBS-D associated with age,occupation,season,and due to emotional and dietary factors induce or aggravate,the duration of illness mainly in1-5years.The middle-aged,fast-paced life of mental workers and retired people lack of exercise were the high-risk population.The disease occured more in summer,we should popularize related knowledge to the vulnerable patient group,let them raise awareness of the characteristics of the disease,pay attention to mental adjustment,keep regular diet,and then reduce the incidence and recurrence of the disease.2.Stagnation and spleen deficiency syndrome and weakness of the spleen and stomach syndrome constituted the largest proportion in the distribution of TCM syndromes,TCM Syndrome relevant to age,middle-aged people susceptible to stagnation and spleen deficiency syndrome and weakness of the spleen and stomach syndrome,for such patients,on the basis of symptomatic treatment,strengthen psychological counseling,regulating the spleen and stomach function are also important.3.TCM drugs:The main drugs for the treatment of this disease:Poria,Coicis Semen,Atractylodis Macrocephalae Rhizoma,Glycyrrhizae Radix Et Rhizoma,Dioscoreae Rhizoma,Amomi Fructus,Nelumbinis Semen,Citri Reticulatae Pericarpium,Astragali Radix,Lablab Semen Album,Bupleuri Radix,Paeoniae Radix Alba,Ostreae Concha,Platycodonis Radix,Saposhnikoviae Radix,Amomi Fructus Rotundus,Coptidis Rhizoma,Bletillae Rhizoma,Codonopsis Radix,shenqu.Drugs in stagnation and spleen deficiency syndrome were mainly used for relieving Qi Stagnancy in liver and invigorating the spleen Powder.Drugs in spleen kidney yang deficiency syndrome were mainly used for tonifying qi,warm the middle warmer,astringing the intestine,and they were the main ingredients of Regulating the Middle Pill and Ginseng,Poria and Bighead Atractylodes Powder;Drugs in spleen stomach damp heat syndrome were mainly heat-clearing and dampdrying drugs,and they contained the major pharmaceutical composition of Pueraria,Scutellaria and Coptis decoction.4.Drug combination:By analysis of the data mining,the combination of two drugs commonly used for the treatment of IBS-D:Nelumbinis Semen and Coicis Semen,Nelumbinis Semen and Dioscoreae Rhizoma,Nelumbinis Semen and Amomi Fructus,Lablab Semen Album and Glycyrrhizae Radix Et Rhizoma,Lablab Semen Album and Dioscoreae Rhizoma,Nelumbinis Semen and Poria,they were main in compatibility.Commonly used drug combination focused on the comprehensive conditioning of the liver,spleen and stomach,and the large intestine,they mainly reflected the effectiveness of strengthening the spleen and replenishing qi and promoting the circulation of damp to stop diarrhea.
