ObjectiveIn Synopsis of Golden Chamber, diarrhea and dysentery are classified in the category of Xiali Disease. ZHANG inherited the objective of differentiating diseases and valuing spleen-stomach in the Internal Canon of Medicine, which provided important theoretical basis for treatments of pediatric diseases for physicians of later generations as well as guiding significance for treatment of clinical pediatric diseases with traditional Chinese medicine. According to physiological and pathological characteristics in children, routine therapy ideas of pediatric Xiali Disease are briefly explained on the basis of ZHANG’s "Treatise on the Spleen and Stomach".MethodsThe method of literature research is adopted in this paper. Based on ancient and modern literatures and examples of modern clinical applications and by collating therapeutic methods for Xiali Disease in ZHANG Zhong-Jing’s Synopsis of Golden Chamber, rules concerning pathogenesis, mechanism, symptom, classification and therapy for Xiali Disease in ZHANG’s theories and modern clinical applications are summarized and analyzed to further explore the application of Zhongjing’s academic thought of treatment in pediatrics.ResultsAfter collating ZHONG’s ancient literatures, it is suggested that there are understandings and discussions on Xiali Disease, but prescriptions vary a lot and involve many pathogeneses without detailed information and that Zhang created his own system of syndrome differentiation-based treatment. It is concluded that causes of Xiali Disease are frequently related with "six evils" (a Chinese saying, which cover wind, coldness, summer heat, wetness, dryness and pathogenic heat), diet, consumptive physique and emotion; the mechanism is mostly viscera dysfunction caused by abnormal viscera transportation and transformation functions. Xiali Disease majorly affects spleen, stomach and large intestine and is linked with liver and kidney; in addition, spleen and stomach are closely related with physiological and pathological features in children. On the basis of studying on Zhang’s therapeutic principles and methods for Xiali Disease, therapeutic ideas are summarized, while prescription, medicine and syndrome-concerned classic pediatric cases are analyzed and explained in detail.The paper focuses on studying the application of ZHANG’s academic ideas of Xiali Disease treatment for pediatric diarrheal, emphasizing the influence and guidance of ZHANG’s treatment ideas for pediatric physician and analyzing application of ZHANG’s prescriptions in the treatment of pediatric diarrheal from the perspective of overall analysis-based diagnosis and treatment. It consists of three parts, including 1) discussion on evolutions of Xiali Disease name, etiology, pathogenesis, and therapeutic method and principle,2) explanation of ZHANG’s discussion and development on Xiali Disease, which explains Zhang’s ideas concerning etiology and pathogenesis, therapy innovations, and applications of prescriptions of Xiali Disease, summarizes characteristic ZHANG’s prescriptions applied by pediatric physicians of later generations, and 3) summarization on specific applications of therapeutic methods and prescriptions in pediatric diarrhea from practical angle of clinical treatment and explanation on current treatments for pediatric diarrhea under the influence of ZHANG’s ideas concerning treatments of Xiali Disease.ConclusionsDevelopment law and historical evolution of Xiali Disease are systemically and rationally organized by collating, summarizing and studying Xiali Disease-concerned data extensively. Its development is studied along different historical stags from aspects of disease name, etiology and pathogenesis, and therapeutic method and principle, ZHANG’s treatment ideas are summarized, and the influences on pediatrics is presented to achieve clear understandings on treatment of pediatric diarrhea, to provide valuable literature concerning application of ZHANG’s prescriptions in pediatric diarrhea and to provide powerful theoretical supports for developments of clinical practices.
