Research objectives:1. According to the clinical curative effect observation of 20 cases of chronic ischemic renal disease patients of blood stasis syndrome, treatment group is given traditional Chinese medicine intervention, in order to clarify the clinical curative effect of the prescription of eliminating stagnation to activate meridians in the aspect of TCM symptom integral and protection of renal function towards chronic ischemic nephropathy.2. According to the establishment of chronic renal ischemia model in which left kidneys of 30 rats are not completed ligated and TCM intervention is given, the influence of the prescription of eliminating stagnation to activate meridians in rat kidney pathological changes and related protein expression is observed, in order to clarify the effect and mechanism of the prescription.Research methods:1. Clinical research methods:20 collected cases of patients with chronic ischemic renal blood stasis were randomly divided into treatment group and control group, both treated by western medicine, the treatment group was given TCM intervention, and the TCM symptom integral and the change of blood pressure, kidney function were observed in two groups of patients before and after the treatment.2. Animal experimental methods:30 rats were randomly divided into control group, the TCM treatment group and model group, TCM treatment group and model group were given insufficient ligation of left renal artery to copy ischemic nephropathy model. Control group was the same as TCM treatment group except not operating renal artery ligation. Three groups were lavaged from the next day after the operation, including control group and model group lavaged with saline, TCM treatment group lavaged with the prescription of eliminating stagnation to activate meridians, and the left kidneys in rats were resected four weeks after the operation. The renal pathological changes were observed by HE and Masson staining, and pathological changes were evaluated by pathological criteria; the expression of MMP-2 and TIMP-2 were detected immunohistochemically.3. Statistical methods:SPSS 20.0 statistical software were applied to do statistical processing and analysis.The results of the study:1. According to the clinical observation, the prescription of eliminating stagnation to activate meridians can improve TCM clinical performance of the patients with chronic ischemic renal disease of blood stasis, and reduce serum creatinine and 24 h urine protein, delay progression of renal dysfunction.2. According to the animal experiments, the prescription of eliminating stagnation to activate meridians can significantly reduce the pathological grading of the ischemic renal damage in the rat kidney, delay the progress of renal interstitial fibrosis, and its mechanism may be related to the increase of MMP-2 and the decrease of the expression of TIMP-2.Research conclusions:The prescription of eliminating stagnation to activate meridians has obvious effect in treating ischemic nephropathy based on clinical observation and animal experiment, therefore it can be used as a basic prescription in clinical practice, along with addition and subtraction.
