| PurposeVentricular contractions (premature ventricular contraction PVC), also known as ventricular premature beat(VBP), is earlier than the basic rhythm (mostly sinus rhythm) appear early ventricular impulses. Currently, in the treatment of Western medicine rely mainly on taking antiarrhythmic medicine, but significant side effects of these drugs, there is a big effect also individual differences; radio frequency catheter ablation, although with widely used, but indications demanding and expensive range of applications limited. Traditional Chinese medicine in the treatment of ventricular contraction,from a holistic approach, a combination of differentiation with disease, less side effects, relatively safe. Tutor Professor Yang Guohua think ventricular contraction pathogenesis mostly qi and yin deficiency, heart missing support, made for the throbbing, so to replenishes qi and yin,ning heart palpitate given to the therapeutic method;and combined with the "Yellow Emperor" that said:"bitter into the heart", the heart is a "fire", and bitterness is yin, pure heart qi, using self-made Si Shen Yang Xin decoction combined bitterness drug in clinical,it can relax nerves nervous, balance of Yin and Yang, and treat ventricular contractions. Based tutor’s clinical experience, observe the curative effect of Si Shen Yang Xin decoction combined bitterness drug in improving ventricular contractions clinical symptoms and anxiety states,in order to objectively evaluate the clinical efficacy, clear features and advantages of the treatment, and provide new ideas for medical treatment of ventricular contractions.MethodsMet the inclusion criteria prior to ventricular contraction in patients using random digit grouping way into the test group and control group. Test group using traditional Chinese medicine, Si Shen Yang Xin decoction combined bitterness drug, one daily, two times a day; control group using western medicine, to oral mexiletine hydrochloride tablets, each 100-200mg, day 3 times; the treatment lasted 28 days, seven days,14 days after taking the 28-day observation point record CRF tables, including the patient’s general condition, Holter monitoring, traditional Chinese medicine syndrome, anxiety Hamilton score and other data, dynamic observation of clinical symptoms, syndromes and anxiety improve the situation before and after treatment to evaluate the clinical efficacy of TCM treatment before ventricular contraction.Statistical results using descriptive statistical analysis, qualitative indicators to frequency tables, percentages description; quantitative indicators were expressed as mean±standard deviation to describe, using t test or rank sum test was used for statistical analysis to determine whether the differences are significant; count index by the number of frequencies, percentages described test using chi-square test. All data using SPSS 22.0 statistical package for analysis, P<0.05 was considered statistically significant.Results1. Baseline analysisTwo groups of patients in general, the ventricular contraction number of Holter,the left ventricular ejection fraction, the symptom score table, the Hamilton Anxiety Scale baseline comparison between pieces of individual points and total packet statistics show that the two groups was no significant difference between (P> 0.05).2. Efficacy analysis2.1 The overall efficacy analysisAfter treatment, the overall effect was observed, the total efficiency of 85.30% experimental group, the control group, the total efficiency of 63.60%, higher than the total effective experimental group, with significant difference (P<0.05). Syndrome aspect, the total efficiency of 91.20% experimental group, the control group, the total efficiency of 72.70%, higher than the experimental group, the two groups was statistically significant (P<0.05).2.2 The number of ventricular contraction ComparisonAfter treatment, significantly reducing the total number of early chamber groups, chamber test group than the control group is more obvious, with significant difference (P<0.05), ventricular bigeminy two groups, ventricular triple law and paired ventricular premature the number of before and after treatment difference was statistically significant (P<0.01), no statistically significant difference between the two groups (P> 0.05).2.3 Analysis of the Hamilton Anxiety ScaleAfter treatment, the total efficiency of 94.20% experimental group, the control group, the total efficiency of 90.90%, higher than the experimental group, but there was no significant difference (P> 0.05); and in terms of clinical recovery, the experimental group accounting for 23.50% clinical cure, clinical cure group accounted for 6.10%, the test group was significantly higher, and there is significant difference (P<0.05).2.4 Left ventricular ejection fraction (EF) analysisAfter treatment EF values were higher in the experimental group and the control, both before and after the two groups was significant difference (P<0.05), in which the test group differences more obvious; there was no significant difference between the two groups (P> 0.05). EF mean in both groups improved within the normal range, so the test group and the control group of Chinese medicine for premature ventricular contractions improvement of cardiac function, whether a clear clinical significance remains to be further research and observation.ConclusionSi Shen Yang Xin decoction combined bitterness drug into the treatment of ventricular contraction has curative effect, there is always more efficient than the comparison group, and can effectively reduce ventricular contractions; for heart palpitations, chest tightness, dry mouth, insomnia and more dream of syndromes improved significantly better than the comparison group; also a clear effect in terms of improving the state of anxiety; heart function improvement, clinical efficacy needs further observation. Si Shen Yang Xin decoction combined bitterness drug of the former Chinese medicine treatment of ventricular contraction, while improving anxiety symptoms associated with it, providing new ideas and methods. |
