| ObjectiveTo inherit the experience of famous veteran teran doctor of TCM and provide reference for clinician in making prescriptionsMethodsThe paper, by practicing with Professor Feng Xinghua, taking notes, collecting proved cases, retrieving and reading relevant literatures and books, talking and discussing with teacher, writing learning experience, has preliminarily analyzed the clinical experience of Professor Feng Xinghua in using Qinlianpingweisan to treat diseases. At the same time, the paper also reviews the TCM cognition on etiology and pathology and treatment of dampness-heat in the spleen and stomach syndrome by document retrieval and summarizes the application of Qinlianpingweisan in modern clinic.Results1. The application of Qinlianpingweisan in the modern clinic:It is mainly used in the treatment of dermatosis and gastrosis stomach. In other respects such as metabolic diseases it is also applied.2. The composition and efficacy of Pingweisan have been greatly expanded from firstly documented in Jianyaojizhongfang to treat stomach qi disharmony to Qinlianpingweitang recorded in Yizongjinjian to treat yanwochuang caused by dampness-heat.3. Professor Feng Xinghua’s understanding of dampness-heat in the spleen and stomach syndrome:The tutor thinks that etiology of dampness-heat in the spleen and stomach syndrome are dampness-heat external contraction, dietary irregularities and the internal impairment of emotion. The spleen provides the materiel basis of the acquired constitution and it is the source of generating qi and blood.Spleen governs movement. Therefore the digestion and absorption of essential substances from water and cereals in diet depends on spleen and stomach, which also transports essence of water and grain to the whole body to maintain the life activities. When spleen and stomach are affected by dampness and heat, the function of spleen and stomach can be damaged. Then there can be a lot of disease. As so called:"Internal injury of spleen and stomach causes various diseases."At the same time, dampness-heat has its unique characteristics. Dampness is lingering. When heat and dampness are entangled, they steam in the spleen and stomach leading to stagnation of the functional qi. The evil can go up with the meridian and collateral, flow down and affect other viscera and bowels. It can also affects skin, oral cavity and joint to show symptoms.4. Professor Feng Xinghua’s ideas to Qinlianpingweisan:Qinlianpingweisan is firstly documented in Yizongjinjian. It developed from Pingweisan. The tutor thinks it can clear heat, dry dampness and regulate qi effectively. Cangzhu and Huanglian are sovereign medical. Cangzhu which is pungent, bitter and warm can dry dampness to fortify the spleen. Huanglian is bitter and cold. It can not only clear heat and dry dampness, but also clear heart fire. As so called:"All kinds diseases with pain, itching and sore are exclusively related to the heart." Therefore Huanglian has a good effect on erythra, acne and mouth ulcer etc. Houpo which is pungent, bitter and warm can move qi to resolve dampness. Huangqin is bitter and cold. It can clear heat and dry dampness, purge fire and detoxify to help Huanglian to play the effectiveness. Both of above are minister medicinal. Chenpi is assistant medicinal to dry dampness to fortify the spleen and regulate qi. It can help Cangzhu and Houpo to play the effectiveness. Gancao is courier medicinal which can coordinate the drug actions of a prescription. When it is unprocessed, it can clear heat and detoxify. All medicines above play roles in clearing heat, drying dampness and regulating qi. Syndrome differentiation and treatment is the method of applying Qinlianpingweisan. Besides the common symptoms, tongue manifestation is the point in syndrome differentiation. It is red tongue and yellow thick fur in general. Two points, connection of differentiation of disease and differentiation of pattern and identifying different syndromes relating to different stages, should be paid attention when Qinglianpingweisan is applied in the treatment of rheumatism.5. Professor Feng Xinghua uses Qinlianpingweisan to treat all kinds of rheumatism and other diseases:In the influence and guidance of "holistic view", "treatment based on syndrome differentiation theory," in Huang Di’s Canon of Medicine and "treating different disease with the same method" in Synopsis of Golden Chamber, the tutor uses Qinlianpingweisan to treat all kinds of rheumatism and other diseases. It can be used as long as there is dampness-heat in the spleen and stomach syndrome. Connection of differentiation of disease and differentiation of pattern and identifying different syndromes relating to different stages should be paid attention when it is applied. It is often used in combination with other prescriptions. Finally the paper sums up the experience of tutor in using Qinlianpingweisan to treat all kinds of diseases, including Behcet disease, SAPHO syndrome, pseudotrichinosis, paracmasis of gouty arthritis, systemic lupus erythematosus, erythema nodosa and oral mucosal diseases. The tutor uses Qinlianpingweisan to treat various rheumatism innovatively to expend the efficacy of it and obtain satisfactory therapeutic effects. It fully reflects the holistic view, treatment based on syndrome differentiation theory and treating different disease with the same method in TCM.Conclusion1. Qinlianpingweisan is mainly used in the treatment of dermatosis and gastrosis stomach. In other respects such as metabolic diseases it is also applied.2. The composition and efficacy of Pingweisan have been greatly expanded during the development from Pingweisan to Qinlianpingweisan.3. The guiding ideology of Professor Feng Xinghua in using Qinlianpingweisan is holistic view, treatment based on syndrome differentiation theory, treating different disease with the same method, connection of differentiation of disease and differentiation of pattern and identifying different syndromes relating to different stages.4. Professor Feng Xinghua creatively uses Qinlianpingweisan to treat all kinds of rheumatism diseases and obtain satisfied effects to expand the clinical application of Qinlianpingweisan. |
