Background and objectiveAs the development of society and life style, the cardiovascular disease risk rises year by year, becoming one of the main death factors domestically and threatening human health as well as life quality. An increasing amount of epidemiological studies have shown that sleep deprivation is highly associated with cardiovascular diseases. However, the overwhelming majority of these studies are lab-based by means of exploring changes of cardiovascular risk parameters or proinflammatory biomarkers after acute sleep deprivation, while population-based studies is scarce in this context. So it’s necessary to operate population-based and clinical research relating to chronic sleep deprivation.Various cardiovascular diseases, including myocardial infarction, stroke and so on, are essentially vascular wall change. Researchers gradually realize that the change of vascular wall is the basis of many cardiovascular diseases. The early discovery and intervene of vascular change clinically are important methods to control cardiovascular disease. The vascular stiffness is believed an independent risk factor and becomes a popular research point. Brachial-ankle pulse-wave velocity (BaPWV) and Ankle-brachial Index(ABI) have been proven as new and noninvasive indicators of the arterial stiffness, which are widely implicated in clinic nowadays and adopted in this experiment.The nurse is chosen as the research object. It is a special occupation which requires night shift work to take care patients day and night, which needs frequent night work and is easy to cause acute sleep deprivation that turns to chronic sleep deprivation when it lasts long. The purpose of our study is to observe the influence of chronic sleep deprivation to cardiovascular diseases through investigating the relationship between the duration of night shift work and arterial stiffness in nurses.MethodsWe conducted a prospective study, enrolling 148 nurses working in Guang’an men hospital from June to September,2013, in Beijing, China, data from which were observed. The baseline characteristics including age, height, weight, years of night shift work, night work times in every year, smoking, alcohol and past medical history were collected. Blood pressure, BaPWV and ABI were measured by OMRON BP-203RPEIII arteriograph system, classified into three categories:normal,mild hard, hard. At the same time, the blood sample was collected to test total cholesterol(TC), triglyceride(TG), low-density lipoprotein cholesterol(LDL-C), glucose(GLU). Choose appropriate methods for statistic analysis, that is ANOVA test and non-parametric test for difference of measurement data, x2 test for enumeration data and ANCOVA test to find independently relationship between night shift work year and arterial vascular condition after adjusting the influence of age, TC, TG, LDL-C, GLU.Results205 nurses in total were enrolling in this experiment and finally 148 nurses were observed after removing male and data-missing objects with the median age 31 years and median BMI 21.94. Divided study subjects into 3 groups, respectively short-term group (the night shift work year<10), middle-term group (10= night shift work year<20) and long-term group (night shift work year=20). The statistic analysis shows significantly statistic significance of baPWV and ABI between groups which in proportion to night shift work year(p<0.001). Factors having the closest relationship with baPWV and ABI were analyzed the night shift work year and the BMI. After adjusted by important factors associated with artery stiffness, night shift work years were independently associated with arterial stiffness (p<0.001).ConclusionsThe night shift work duration of nurses in Guang’an men hospital is independently associated with their arterial stiffness condition and longer the time duration is, more significant the influence to blood vessel is. The population-based clinical research proves that chronic sleep deprivation is an important risk factor of cardiovascular diseases.
