ObjectiveA randomized control clinical study, Four Ginsengs Heart-Tonifying Decoction (Si Shen Yang Xin Tang) improve mild to moderate coronary artery stenosis and clinical symptoms of anxiety and depression were used to observe short-term effect. This study was to clear the Four Ginsengs Heart-Tonifying Decoction (Si Shen Yang Xin Tang) in improving the characteristics and advantages of mild to moderate coronary artery stenosis and the clinical symptoms of anxiety and depression. And to Provide objective basis for the long-term clinical application and extension of the decoction.MethodAll cases were collected from the Department of internal medicine and emergency internal medicine of Wangjing Hospital, Chinese Academy of traditional Chinese Medicine from July 2015 to January 2016. This study included 66 patients with mild to moderate coronary artery stenosis according to the implementation plan. All patients were randomly divided into the treatment group and the control group. There were 3 cases lost during the treatment,31 cases in the final treatment group and 32 cases in the control group. The treatment group received routine Department of internal medicine therapy, with Four Ginsengs Heart-Tonifying Decoction (Si Shen Yang Xin Tang), daily a dose 2 times daily. The control group was treated with basic therapy in routine Department of Internal Medicine. Conventional therapy is nitric acid ester, statins, beta blockers, calcium channel blockers and anti platelet drugs. The course of treatment was 2 weeks. ECG examination was performed after one week of treatment. After two weeks of treatment, ECG, TCM Syndrome Scale, Hamilton anxiety scale (HAMA) and Hamilton depression scale (HAMD) were evaluated, and compared with before treatment. To all the cases in accordance with the treatment plan, good compliance, and no other relevant treatment during the test were analyzed statistically. Statistics is done by using SPSS20.0 software. Measurement data is done by mean plus or minus standard deviation (x±s) for statistical description. Measurement data of different groups were compared with t test or non parametric test. The count data was compared by X2 test. there is statistical difference by p< 0.05.Results1 Baseline analysisThe two groups in age, gender, disease duration, vital signs (blood pressure, respiration, heart rate), coronary artery stenosis, the degree of anxiety, depression, and each item before the therapy of TCM syndrome integral and Hamilton anxiety scale and Hamilton depression scale index and total score were compared. By t test, nonparametric test or X2 test, the two groups had no statistically significant difference (p> 0.05) to study the control.2 Curative effect analysis2.1 Analysis of ECG improvementAfter one week treatment, the total effective rate of the treatment group was 42%. The total effective rate of control group was 31.3%. There was no significant difference between the difference between two groups (P> 0.05). After two weeks treatment, the total effective rate of the treatment group was 83.9%, higher than that of the control group, the total effective rate was 56.3%. The difference of curative effect between the two groups was statistically significant (p< 0.05).2.2 Improvement of dynamic electrocardiogramAfter two weeks of treatment, in the treatment group, the total effective rate of 77.4%, higher than that of the control group, the total efficiency was 50.0%, two group differences in efficacy by comparison with statistical significance (P< 0.05).Treatment group in the treatment of frequency of myocardial ischemia, myocardial ischemia continued time, total ischemia burden total and better than that of the control group, the index difference has statistical significance (P< 0.05).2.2 Efficacy analysis of TCM syndrome scoreAfter two weeks treatment, the total effective rate of the treatment group was 87.1%, higher than that of the control group, the total effective rate was 59.4%. The difference of curative effect between the two groups was statistically significant (p< 0.05).The treatment group was better than that in the treatment of chest tightness, shortness of breath, palpitation, fatigue, dizzy, insomnia and other aspects of the control group. The difference was statistically significant (p< 0.05). There are no differences (p> 0.05) in the treatment of chest pain and bad facial color of the two groups.2.3 HAMA efficacy analysisAfter two weeks treatment, the total effective rate of the treatment group was 48.4%, higher than that of the control group, the total effective rate was 21.9%. The difference of curative effect between the two groups was statistically significant(ρ< 0.05).In the Treatment group, compared with the control group, the differences of the indexes of anxiety mood, tension, cardiovascular system and respiratory symptoms were statistically significant (ρ< 0.05). Two groups in the fear of treatment, insomnia, memory or attention disorder, depressed mood, symptoms of muscle system, feeling system symptoms, gastrointestinal symptoms, genito urinary symptoms, autonomic symptoms, talks behavior etc. no difference (ρ> 0.05).2.4 HAMD efficacy analysisAfter two weeks treatment, the total effective rate of the treatment group was 41.9%, higher than that of the control group, the total effective rate was 3.2%. There was a significant difference between the two groups [ρ< 0.05).In the Treatment group, mental anxiety, physical anxiety index, hypochondriasis were better than the control group. The index difference has statistical significance (ρ< 0.05); Two groups in the treatment of depression, guilty sense, suicide, difficulty falling asleep, sleep is not deep, wake up early, work and interest, slow, agitation, gastrointestinal symptoms, systemic symptoms, weight loss, insight etc. aspects no difference (ρ> 0.05).ConclusionFour Ginsengs Heart-Tonifying Decoction (Si Shen Yang Xin Tang) Combined with western medicine treatment mild to moderate coronary artery stenosis of overall curative effect is superior to western medicine in the treatment of, and has a good safety. Four Ginsengs Heart-Tonifying Decoction (Si Shen Yang Xin Tang) to significantly improve the mild to moderate coronary artery stenosis in patients with anxiety and depression, and has good security.
