| At present, as the safeguard system that our country provided to doctors is imperfect and the contradictions between patients and doctors intensified gradually, doctors work under great occupational stress. along with the reform of public healthy, the implementation of evidence-presenting inversion system and the enhancement of law consciousness of patients, the expected value of patients and the society to doctors raised, they would experienced more stress. Clinician usually directly contacts with the patient, is responsible to work out the therapeutic schedule and the main medical practice, in view of the fact that medical procedure uncertainty, high risk and so on, their working pressure relatively is bigger in the medical personnel community. Clinician's working pressure will directly not only affect their own physical and moral integrity, also further affect the medical quality and doctor-patient relationship. Therefore to investigate and research clinician's pressure condition and occupational stressors systematically, are helpful for stress management, thus maintains clinician's physical and moral integrity , improves the medical quality and doctor-patient relationship. But present, researches focusing on the clinician's occupational stress are fewer in our country. So it is necessary to compile a scientific and canonical tool to measure their job stress.The research takes all levels of shanghai public hospital clinical doctors as investigate objects. Based on the references to massive literature material both at home and abroad, interview with clinicians and consult with psychological professors as well, a scale named"Scale for Occupational Stressors on clinician"is developed, which composed of 38 items after repeated measurement and revise. Then take the scale to measure 811 clinicians from shanghai all levels of public hospitals.The statistical test showed:1."Scale for Occupational Stressors on clinician"is composed of 7 factors: organization and management, vocational interest, work load, career development, interpersonal relationship, external environment and doctor-patient relationship, which achieved good to excellent levels of construct validity and content validity, as well as internal consistency,so it can be used in job stress researches of clinicians.2 .The characteristics of clinicians' occupational stress in shanghai area:1).In general, the clinicians' feeling of occupational stress is relatively strong.2). Clinicians in different levels of hospitals faced work stress is significantly different, which in the second-grade hospitals is the highest, next is in the community health services centre, the lowest is in the third-grade hospitals.(3)The occupational stress is remarkable different among clinicians with different educational background, the pressure that university record clinicians experienced is maximal, that graduate record clinicians experienced is relatively minimal.4) There is notable difference in the overall stress feeling, workload and external environment, considering to different sex, the male is higher in the overall stress feeling and workload, rather the female is higher in external environment.5).the overall work stress is notable different among clinicians with different the titles of technical post, the pressure of clinicians having intermediate title of technical post experienced is highest, the lowest is that with senior title of technical post.6).Among the groups of work year, the 5-10 years group gets the heaviest job stress than the group of above 10 years and the group of less than 5 years, the difference is significant.7).There is notable difference in different specialty, the pressure of doctors of obstetrics and gynecology, pediatrician is highest, and the doctors of dermatology, ophthalmology, Chinese traditional medical and other specialties have the lightest stress. |
