ObjectionsTo analyze the situation of tuberculosis control during the period from 2002 to 2008 in Xuchang, Evaluate the effect of modern TB controlling strategly (DOTS), find the factors which could elevate the discovering rate of tuberculosis, and provide a basis of effective approach of TB controls.MethodsTo analyze the prevalence of TB in Xuchang city by collect the monthly, quarterly, and annual reports of TB control from 2002 to 2008; To analyze the impact on the detection rate of patients by collect the final assessment of TB control project, and count the amount of annual funding, health education, medical anti-cooperation, business training and supervisory management.The results were analyzed with Excel2003, Epidate3.1 and SPSS17.0.Results1.Since Xuchang city started the TB control project of world bank/united kingdom grants in 2002,until 2008 the strategly of DOTS covereged of 100%.2. The annual rate of record patients had increased year by year form 2002 to 2005, from 98.01/100,000 rose to 343.04/100,000, stabilized after 2006.3. The number of smear-positive TB patients had increased year by year, the rate of new smear positive was 14.43/100,000 in 2002, and the new smear positive patients was 28.86%, the registration rate of new smear positive patients and the detection rate of new smear positive patients were 37.36/100,000,74.88% in 2008, That was two times more than in 2002, the new smear-positive detection rate of patients achieved 70% after 2005.4. The proportion of early retreatment has fallen steadily from 2002 to 2008 of 1:0.31 to 1:0.06 declined; the proportion of smear-negative smear-positive has fallen steadily; registration rate of new smear-negative patients increased year by year from 2002 to 2005, and declined from 2006 to 2008.5. Over the years, male patients of the new smear-positive recorded registered were more than female patients significantly, the pealcage for male was 25-65 year-old, yet women was 15-65year-old groups, the ratio between the age compositions of each year was not significantly different6. To Smear-positive tuberculosis patients the negative rate of the end of three months was higher than the end of two months, and the differences between those had statistically significant.7. The average cure rate of smear-positive patients for first treatment was 90.84%, and the average cure rate for smear positive retreated patients was 81.36%.8. The relation between funding for TB control and the new smear positive was a positive correlation, r=0.876 (r2=0.786, P<0.01),the regression equation was Y=16.536X+4.066.9. The relation between health education and the new smear positive detection rate of patients was a positive correlation, r=0.969, (r2=0.940, P<0.01), Y=77.938X+8.828.10. The relation between the number of training medical personnels on patients and the detection rate of patients was a positive correlation, r=0.964, P<0.001; the relation between steering rate of patients and the detection rate of patients was a positive correlation, r=0.523, P=0.229, and it had not the statistical significance.ConclusionUnder the guidance of the project, the DOTS coverage was 100% in Xuchang, new smear-positive detection rate was 70%, and the cure rate of smear-positive patients was more than 85%; the detection rate of new smear positive patients was rised obviously. Funding for TB prevention, control, health education, and the training of medical personnel played the important roles in the detection rate on TB. The quality of smear-positive TB patients supervised fully should be further improved. The support of the project on TB control is obvious.
