Objective : To discuss the results of Severe Chronic Compressive Cervical Myelopathy and study relationship between gender,duration of symptoms,T2 signal change,age,number of involved level,operation time and canal-body ratio(CBR)with treatment result. Method : 68cases with Severe Chronic Compressive Cervical Myelopathy treated by decompression surgery was reviewed. Relationship between gender,duration of symptoms,T2 signal change,age,number of involved level,operation time and canal-body ratio(CBR)with their treatment results were statistical analyzed. Result:The JOA scores rise markedly after operation,Paired t-test for dependent samples is used to estimate the difference,there is statistical difference of JOA scores between Preoperation and Postoperation(t=-23.89,P=0.00). The treatment result between two groups of different gender has no statistical differences(t =-0.96,P=0.33),The treatment result between two groups of different duration of symptoms has statistical differences(t=2.52,P=0.01),The treatment result between two groups of different T2 signal change has statistical differences(t =6.13,P=0.00),The treatment result among of three age groups has statistical differences(F=19.53,P=0.00),The treatment result among of three groups of different number of involved level has no statistical differences(F=2.11,P=0.13),The treatment result between two groups of different operation time has no statistical differences(t=-0.68,P=0.49)and The treatment result between two groups of different Canal-body ratio(CBR)has statistical differences(t=5.40,P=0.00). Conclusion Decompression surgery can be a reasonable treatment option for Severe Chronic Compressive Cervical Myelopathy. The operation result has relationship with disease course,T2 signal change,age and CBR. In a certain degree,even some low JOA scores Patients with short disease Course,no T2 signal change,younger age and larger CBR whom can be Performed operation as soon as Possible,the Satisfactory result of treatment also can be obtaind. The operation result has no relationship with number of involved level,gender and operation time but it should be continued to study.
