Objective: To compare and analyze the clinical and pathological characteristics of invasive lobular breast cancer (invasive lobular carcinoma, ILC) and invasive ductal breast cancer (invasive ductal carcinoma, IDC). Methods: Retrospectively analyze 69 cases and 457 cases of operable patients with primary pure ILC and IDC, confirmed by histopathology in Breast Surgery department of Xinjiang tumor Hospital from January,2004 to December,2009.The following clinicopathologic features were analyzed, including patients' age distribution, menstrual status, imaging appearance, tumor location, tumor size, lymph node metastasis, and the expression of immunohistochemistry. Results: the median age of ILC was 50,and IDC was 47. In the age group≤35,36-65,and≥65 proportion of ILC was 2.9%,81.2%,15.9%, respectively, IDC was 9.2%,79.6%,11.2% respectively(P>0.05).The proportion of premenopaual patients was 50.7% and 60.8% respectively in group ILC and IDC (P = 0.111). The detection rate only by ultrasound or mammography was 79.2%,73.6% in group ILC ,and 92.0%, 84.5% in group IDC(P<0.05).The proportion of the synchronous bilateral primary breast cancer in ILC and IDC was 5.8% and 0.4% (P=0.000), the proportion of primary tumor located in the central area was 34.3% and 15.6%, respectively (P=0.001) .The proportion of multicentre, mulifocality was 13.4%, 2.4% respectively ,(P=0.000). The proportion in tumor size T4 of group IDC and group ILC were 8.7% and 0.9% (P=0.000), Lymph node metastasis rates were 52.9%,and 46.1% respectively in these two groups (P=0.295), The proportion of the number of lymph node metastasis in group≥10 in the ILC was 20.6%, and IDC was 9.3%(P=0.005).The positive expression of ER,PR,C-erbB-2 was 66.7%,56.5%,14.5% in group ILC and 58.2%,57.1%,22.3% in group IDC(P=0.182,0.926,0.139).Conclusion:1. The imaging findings of ILC were not always typical compared with IDC.combination of ultrasound and X-ray mammography examination may significantly improve the detection rate and reduce the rate of misdiagnosis. 2. ILC had a higher incidence of synchronous bilateral primary breast cancer,Primary tumors of ILC more located in the central area and appeared more multicentre and mulifocality compared with IDC. 3. The primary tumor size of ILC was larger than IDC, and ILC had more positive lymph nodes metastasis than IDC.
