Objective:This study used an animal model of Severe acute pancreatitis(SAP) to examine the effects of CO2 pneumoperitoneum on SAP and lung tissue,lung NF-KB and ET/NO in serum in rabbits, as in laparoscopic treatment of SAP applications based on the theory.Methods:30 healthy Zelanian rabbits were randomly divided into 3 groups: (1) CO2 pneumoperitoneum group,which include 10 rabbits. SAP complicated with renal insufficiency was induced by injection of 5 % sodium taurocholate through retrogradely pancreatic duct via duodenal papilla with epidural catheter,closed puncture hole with medical sealant glue. Then six hours later gave CO2 pneumoperitoneum at a pressure of 12 mmHg for 120 minutes. (2) Open abdominal cavity group, which include 10 rats.The animals were treated as same as CO2 pneumoperitoneum group except CO2 pneumoperitoneu. (3) Control group, sham operation, open abdomen for 30 minutes, and then the abdomen was closed. when gave CO2 pneumoperitoneum concluded then killed the CO2 pneumoperitoneum group animals immediately.eight hours after the surgical procedures, the remains were killed .Cut pancreatic and lung tissue histopathologic examination pathology optical microscope and NF-кB expression index. The artery blood samples were gathered for examining serum PO2andPCO2;The vein blood samples were gathered for examining serumNO,ET and AMY. Comparison and analysis of three sets of experimental data.Results:(1) Ordinary light microscope about pancreatic pathology who have Pathology change of CO2 pneumoperitoneum group and open abdominal cavity group show that there are focal pancreatic necrosis , inflammatory cell invasive and hemorrhage. Ordinary light microscope about pancreatic pathology of Control group shows that there are not focal pancreatic necroses, inflammatory cell invasive and inflammatory reaction around blood vessel. After statistics the pathology scores of three groups,the result show: The CO2 pneumoperitoneum group(10.1±1.64) and the open group(9.3±1.31) were significant difference compare to the control group(1.00±0.05),and have statistical significance (P < 0.05). There was no statistically important differences between CO2 pneumoperitoneum group and the open group (P =0.294).Ordinary light microscope about lung pathology who have Pathology change of CO2 pneumoperitoneum group and open abdominal cavity group show that there are a lot of neutrophils,little angiotelectasis,edema fluid seepage from lung and inflammatory cells ,exudation in bronchial,degeneration and necrosis of bronchial epithelial cell,bronchial and surrounding tissues to be instinct with inflammatroy cells;Ordinary light microscope about lung pathology of control group show normal. After statistics the pathology scores of three groups,the result show: The CO2 pneumoperitoneum group(4.67±1.22) and the open group(4.30±1.25) were significant difference compare to the control group(1.70±0.82),and have statistical significance (P < 0.05). There was no statistically important differences between CO2 pneumoperitoneum group and the open group (P=0.85).(2) The wet to dry lung weight ratio (W/D) were significantly higher in CO2 pneumoperitoneum group(5.03±1.47) and the open group (4.69±1.73)compared to the control group (1.56±0.85)(P <0.05). There were no significant differences between CO2 pneumoperitoneum groups and the open groups (P=0.105).(3) The concentration of AMY in serum: CO2 pneumoperitoneum group: (2856±730); open abdominal cavity group: (2619±693);control group: (422±91). Results analysised by ANOVA check discovers: data in CO2 pneumoperitoneum group and open abdominal cavity group have statistical significance to control group (P < 0.05); data in CO2 pneumoperitoneum group has no statistical significance to open abdominal cavity group (P=0.174).(4)The concentration of ET/NO in serum: CO2 pneumoperitoneum group: (1.43±0.08); open abdominal cavity group: (1.39±0.06); control group: (0.91±0.03). Results analysised by ANOVA check discovers: data in CO2 pneumoperitoneum group and open abdominal cavity group have statistical significance to control group (P < 0.05); data in CO2 pneumoperitoneum group has no statistical significance to open abdominal cavity group (P = 0.312).(5) The concentration of NF-кB expression index: CO2 pneumoperitoneum group: (0.38±0.12); open abdominal cavity group: (0.44±0.14); control group: (0.19±0.08). Results analysised by ANOVA check discovers: data in CO2 pneumoperitoneum group and open abdominal cavity group have statistical significance to control group (P < 0.05); data in CO2 pneumoperitoneum group has no statistical significance to open abdominal cavity group (P=0.324).(6) PO2 of arterial blood CO2 pneumoperitoneum group: (74.5±2.3); open abdominal cavity group(76.1±1.7); control group (98.0±4.1). Results analysised by ANOVA check discovers: data in CO2 pneumoperitoneum group and open abdominal cavity group have statistical significance to control group (P < 0.05); data in CO2 pneumoperitoneum group has no statistical significance to open abdominal cavity group (P=0.205).(7) PCO2 of arterial blood CO2 pneumoperitoneum group: (54.30±3.4); open abdominal cavity group(52.5±2.0); control group (40.2±2.2). Results analysised by ANOVA check discovers: data in CO2 pneumoperitoneum group and open abdominal cavity group have statistical significance to control group (P < 0.05); data in CO2 pneumoperitoneum group has no statistical significance to open abdominal cavity group (P=0.173).Conclusions:CO2 pneumoperitoneum may without any effect to SAP rabbit's lung tissue,lung NF-KB and ET/NO in serum...
