Clinical Analysis Of Acute Abdomen Combined With Gestation

Acute Abdomen is one kind of disease that mainly shows acute abdominal pain. It requires early diagnosis and first aid. Acute Abdomen contains acute appendicitis,acute cholecystitis, gallstone, acute pancreatitisand intestinal obstruction so on . Because of the changes of anatomical position and internal hormone,and the lack of clinic characteristic ,acute abdomen combined with gestation is difficult to diagnose,which could result in misdiagnosis and delay in treatment.The mother and fetus is indanger. How to identify and diagnose the acute abdomen has a direct influence to the salty of the mother and fetus. So it is necessary to summarize and analyze the clinicalpresentation,diagnosis , treatment and the reason of misdiagnosis of surgical acute abdomen while pregnancy.Thirty- five cases of acute abdomen combined with gestation during Jan.2000 to Dec. 2009 in our hospital were collected and analyzed to investigate the diagnosis and treatment of acute abdomen combined with gestation .Objective : To investigate the diagnosis ,reatment and the reason of misdiagnosis of acute abdomen combined with gestation .Methods : To collect thirty- five cases of acute abdomen combined with gestation during Jan.2000 to Dec.2009 in our hospital and to make a retrospective analysis.Results : There were thirty-five cases of acute abdomen combined with gestation.Eight cases were early pregnancy.Sixteen mid stage gestation,and eleven late stage. The common acute abdomen while pregnancy was acute appendicitis.There were eighteen cases ; Acute cholecystitis was less than acute appendicitis. nine cases acute cholecystitis;four cases acute pancreatitis;two cases intestinal obstruction; two cases urinary stone。Eleven cases were operated and twenty cases under conservative measures in all cases; There were four threatened premature labor, three premature labor, four termination of pregnancy, and four fetai death in these thirty- five cases. Nomaternal death occured.Five cases were misdiagnosed in the first twenty-four hours, and misdiagnosis rate was 14.3%.Conclusion: The acute abdomen combined with gestation should be thinked highly of. The clinical presentation and laboratory examination are important for early diagnosis and treatment. Consultation of a surgical doctor is necessary for decreasing the misdiagnosis. The common acute abdomen while pregnancy is acute appendicitis.Acute cholecystitis is less than. Surgical intervention should be taken for acute appendicitis,but it should be treated individually.some slight cases suit to conservative measures. Patients with acute cholecystitis, gallstone ,acute pancreatitis, intestinal obstruction and urinary stone should be treated individually.we should firstly choose conservative measures. Reasonable surgical intervention does not increase the frequency of abortion, premature labor, fetal and maternalmortality.
