Objective:The morbidity and mortality of Gastric cancer are higher of other malignant tumors in our country. It is late detected, bad treatment effect and high mortality among a considerable number of patients. Lymph node metastasis is the main way in gastric cancer. It might effectively prolong survival time and reduce mortality if we can restrain lymph node metastasis. Recent studies have found that the increase expression of COX-2 is closely related to the tumor metastasis through lymph node, and then the most important metastasis way in gastric cancer is the lymph node channel. Whether lymph node involved is the foundation of tumor stage and the direct treatment of regional lymph node drainage, and is also an important prognostic indicator. VEGF-C (Vascular endothelial growth factor-C) is a new member of the VEGF/PDGF family and is considered a specific lymphangiogenesis-stimulating factor. It can promote lymphangiogenesis and cells metastasize to regional lymph node in cancer, and can protect lymphatic endothelial cells from apoptosis by serum-inducing. Although there have been many reports about the expression of COX-2 in gastric cancer, the relativity to lymph node metastasis is scarce. We select the gastric cancer and normal gastric mucosa tissue to detect the expression of COX-2 and VEGF-C and lymphatic vessel density (LVD) by immunohistochemical, aim to explore the significance and the relationship with lymph node metastasis of the expression of COX-2 in gastric cancer.Method:Inpatients were elected in department of gastrointestinal surgery in Luzhou Medical College Hospital in 2008-2009, all of the cancer paraffin-embedded tissue samples were confirmed non-cancerous peripheral tissues by pathology, all patients were not treated by chemotherapy and radiotherapy before operation. Reagent:Rabbit anti-human COX-2 polyclonal antibody(United States Santa Cruz Inc); Rabbit anti-human VEGF-C polyclonal antibody,mouse anti-human D2-40 monoclonal antibody,SP kit,DAB color reagent(were purchased from Beijing Zhong Shan Golden Bridge Biotechnology Co.Ltd.). Use of streptomycin-biotin-peroxidase (SP) staining and concrete steps by SP immunohistochemistry kit instructions carried out, Slice set citric acid buffer (PH6.0), the high heat (95℃,13min) fixed antigen. The results to determine:Optical microscope observation, COX-2, VEGF-C positive signals in the cytoplasm or cell membrane yellow or brown granules. COX-2, VEGF-C protein expression level using staining intensity score +positive cells in an area of the integral representation. D2-40 positive expression of lymphatic endothelial cells showed brown. The positive staining of endothelial cells or endothelial cells to form clusters of strip or gap-like structure and a cavity as a lymphatic vessels were counted. Result:1 The expression rates of COX-2 in gastric cancer tissues and cutting edge of gastric mucosa were 61.4%,13.64%.The expression of COX-2 were closely related with degree of differentiation of gastric cancer, gastric cancer TNM staging, lymph vessel density (LVD), lymph node metastasis. (P<0.05)2 The expression of VEGF-C was 56.8%in gastric cancer tissues. The expression of VEGF-C was not related with the degree of differentiation of gastric cancer, (P>0.05) It's expression was significantly correlated with clinical TNM stage, lymph vessel density (LVD), lymph node metastasis. (P<0.05)3 The expression of COX-2 was positively correlated with VEGF-C in gastric cancer.(P<0.05)4 Gastric lymphatic vessel density (LVD) was closely related with the differentiation degree of gastric cancer, clinical TNM stage, lymph node metastasis.(P<0.05)Conclusion:1The overexpression of COX-2 was closely related with tumor differentiation, clinical TNM stage, LVD, lymph node metastasis in gastric cancer, which proved that COX-2 played an important role in the occurrence,development of gastric cancer.2 The overexpression of COX-2 was positively correlated with VEGF-C suggested that they may play a synergistic effect in the lymphatic vessel formation and development of gastric cancer.3 Lymphatic vessel density was correlated with clinical stage, lymph node metastasis, which suggested there were new lymphatic vessels in the development of gastric cancer.
