Impact On QOL Of Patients With Advanced Gastric Cancer Treated By JianPi Tongluo Formula Combined With TCF Regimen Chemotherapy

This task was composed of three parts.First part reviewed literature about advanced gastric cancer treated by Chinese Herbal Medicine in combination with taxanes based regimen chemotherapy. As first, we performed a systematic review of Chinese Herbal Medicine combined with chemotherapy in patients with advanced gastric cancer. Purpose:This systematic review and meta-analysis were performed to assess the efficacy and tolerability of Chinese Herbal Medicine combined with chemotherapy in patients with advanced gastric cancer. Methods:Randomized clinical trials on Chinese Herbal Medicine combined with chemotherapy in advanced gastric cancer were identified by electronic searches of CBM, CNKI, PubMed. Meta-analysis was performed using the fixed effect model. Response rate, Karnofsky performance score and hematological toxicity reported as risk ratio (RR) with 95% CI, was the primary outcome measure. Results:Jadad score of 32 included trials were 1-3 according to modulated Jadad score, and were identified as low quality trials. Meta-analysis of Chinese Herbal Medicine combined with chemotherapy versus chemotherapy only showed response rate(RR=1.35; 95%CI,1.19 to 1.53), Karnofsky performance score (RR=2.01; 95%CI,1.55 to 2.60) and hematological toxicity(RR=0.46; 95%CI,0.28 to 0.75), respectively. Conclusion:Chinese Herbal Medicine combined with chemotherapy exhibit a higher response rate, Karnoforsky performance score and lower rate of hematological toxicity than chemotherapy only. Whereas, because of low methodological quality of included trials, Chinese Herbal Medicine combined with chemotherapy have not been recommended as credible evidences.A review of taxanes based chemotherapy in advanced gastric cancer also was performed in first part.Second part discussed the relations of LuoBing theoretics and gastric cancer.Third part reported clinical trial on JianPiTongLuo formula combined with TCF regimen in advanced gastric cancer. Objective:Randomized controlled clinical trial to observe impact of JianPiTongLuo formula combined with TCF regimen in advanced gastric cancer patients quality of life. Methods:According to pre-test to determine the sample size had been calculated before trial performed and the trial will need to include 58 patients with advanced gastric cancer.58 patients were randomly divided into treatment group and control group,29 patients in each group. The treatment group were treated by JianpiTongluo method combined with TCF regimen, and in the control group were TCF regimen only. EORCT QLQ C-30 V3.0 Chinese version was used to measure quality of life of patients, while response rate, score of TCM symptoms, CEA, CA199, weight, and toxicity were assessed. Results:After treatment, the treatment group had higher score than the control group in the field of physical function, role function, social function, overall quality of life, the difference was statistically significant (P<0.05). The treatment group had lower score than the control group in the field of pain, naupathia, appetite loss, diarrhea, the difference was statistically significant (P<0.05). Score of TCM symptoms, the treatment group was higher than the therapy group, the difference was statistically significant (P<0.05). In the response rate, weight, CEA, CA199 and toxicity, the two groups was no significant difference (P>0.05). Conclusion:JianPi TongLuo formula combined with TCF regimen can significantly improve the quality of life in patients with advanced gastric cancer, without increasing toxicity.
