Objective:By using the methods of percutaneous poking fixation with Kirschner wire(PPFK) and open reduction Internal fixation with plate(ORIF) for Sandersâ…¡type, Sandersâ…¢type calcaneal fractures, observing and comparing the clinical effects of the two modus operandis, to explore the better treatment of calcaneal fractures.Methods:48 cases of the affected foot that met the inclusion and exclusion criteria were collected, according to the modus operandi and the random of endemic area, divided into PPFK(A) and ORIF(B). A randomized controlled study of multi-angle, from the residual pain after the heel surgery, postoperative pain-free walking distance, function scores, Bohler angle, Gissane angle, calcaneal height, width changes, obsering and contrasting the clinical effects in treating the Sanders II type, Sanders III type calcaneal fractures by using the methods of PPFK and ORIF.Results:1 On the incidence rate of residues heel pain, A group and B group had no significant difference (P> 0.05). 2 In the average maximum walking distance of the affected foot that after one year, A group and B group were significantly different (P<0.05), B group is better than A group in the average maximum walking distance3 The Maryland score of postoperative between the two groups:the Postoperative functional scores of two groups were significantly different (P<0.05), the average score of B group was significantly higher than A group; but the both groups had no significant difference on the excellent rate of postoperative Maryland.4 On the Incidence rate of complication, the B group had higher incidence rate of complication than the group A (P<0.05).5 Compared the X-Ray Parameters of preoperative and postoperative one week in each groupA group:preoperative and after one week were significantly different (P<0.05) in Bohler angle, Gissane angle, calcaneal height and width; the X-Ray Parameters of after one week was better than the preoperative.B group:preoperative and after one week were significantly different (P<0.05) in Bohler angle, Gissane angle, calcaneal height and width; the X-Ray Parameters of after one week was better than the preoperative6 X-Ray Parameters between the two groups compared After one week:Compared the two groups in Bohler angle, Gissane angle, calcaneal height and width,there were no significant difference (P<0.05);the B group is better than the A group.After three months:Compared the two groups in Bohler angle, Gissane angle, calcaneal height and width,there were no significant difference (P<0.05);the B group is better than the A group.After one year:Compared the two groups in Bohler angle, Gissane angle, calcaneal height and width,there were no significant difference (P<0.05);the B group is better than the A group.7 Each X-Ray parameter of the two groups postoperative comparisonIn the comparison of Bohler angle, Gissane angle, calcaneal height and width:A group:the four parameters had no significant difference between the After one week and After three months (P<0.05);but there were significant difference between the After three months and After one year (P<0.05)B group:the four parameters had no significant difference among the After one week, After three months and After one year(P<0.05)Conclusion:There were no significant difference on the incidence rate of residues heel pain and the excellent rate of postoperative Maryland Scores between A group and B group;but on the residual pain after heel surgery, postoperative pain-free walking distance, function score, Bohler angle, Gissane angle, calcaneal height, width changes,B group is better than the A group,and there was a certain degree loss on the four Parameters after one year; however, on. the incidence rate of complication, the B group had higher incidence rate of complication than the group A.So if we can control the postoperative complications,the modus operandi of ORIF is a better choice.
