Emergency Treatment For The 187 Patients With Ruptured Hepatic Cystic Echinococcosis

Objective: To discuss and evaluate the curative effect of the surgically emergency treatment for ruptured hepatic cystic echinococcosis. Methods: Our hospital introduced a normative treatment about operation for hepatic echinococcosis in the year 2002, According to it, the ruptured hepatic cystic echinococcosis were emergency treated from 1994 to 2009 in our hospital divided into group A (from 1994 to 2001) and group B (from 2002 to 2009), Summarized their clinical symptoms and surgical treatment manners were retrospective studied and compared. Results: For patients with hepatic cystic echinococcosis rupture into abdomen, the group of Classic internal cystectomy (A1)was higher than improved internal cystectomy(B1) in the postoperative complications of liver residual cavity, time of draining, postoperative hospital stays, recurrence and Celiac growth (P<0.05); operation time, blood loss and thorax hydrocele were not statistically significant between the group A1 and B1 (P>0.05). For patients with hepatic cystic echinococcosis rupture into bile duct, the group of Classic internal cystectomy (A2)was higher than improved internal cystectomy(B2) in the postoperative complications of liver residual cavity, time of draining, postoperative hospital stays and recurrence (P<0.05); operation time, blood loss, thorax hydrocele and celiac growth were not statistically significant between the group A2 and B2 (P>0.05). All 187 cases of ruptured hepatic cystic echinococcosis, three cases(1.6%)died. Conclusions: Patients of ruptured hepatic cystic echinococcosis should do interrelated examination, while treat their shock, infection and hypersusceptibility, and operate immediately after clear diagnosis. Improved internal cystectomy is lower than Classic internal cystectomy in the recrudescence and postoperative complications of liver residual cavity, and so on, curative effect is obvious. Moreover, patients should take medicine after operation, in order to prevent and treat recrudescence.
