Objective: To investigate the risk factors of lymph node metastasis in rectal cancer. Methods: A retrospective analysis was performed in the 210 patients with rectal carcinoma who underwent surgical resection from Jan 1, 2007 to Dec 31, 2009 in Xinjiang Medical University Tumor Hospital. The associations between lymphatic metastasis and Clinicopathological Characteristic were evaluated by Chi-squared test and Logistic regression in SPSS 13.0 for windows. Analyze the influence factor of the lymph node metastasis in rectal cancer. Results: The rate of lymph node metastasis in this series of rectal cancer was 30.5%, In 1 586 lymph node checked, metastases were identified in 150 lymph nodes (9.5%). After entering crosstabs estimation, gender, race, age, CEA level, the diameter of affected bowel, the length of tumor and tumor site were not correlated with lymph node metastasis (P>0.05). Univariate analysis showed that the degree of infiltration, the histological type, gross type of the tumor, differentiation degree were correlated with lymph node metastasis (P<0.05). Multivariate analysis showed that the degree of infiltration, gross type of the tumor and differentiation degree were proved to be independent risk factors of lymph node metastasis in rectal cancer (P<0.05). Logical regression analysis showed that the strength of affecting LNM was decreasing in order of the differentiation degree, the degree of infiltration. Knurl-type and Ulcer-type, Knurl-type and Infiltration-type were compared with each other in gross type of the tumor, the results of each group had statistical significances in lymph node metastasis (P<0.0017). Conclusion: The lymphatic metastasis of rectal carcinoma was related to patients' the degree of infiltration, the histological type, gross type of the tumor and differentiation degree. The degree of infiltration, gross type of the tumor and differentiation degree were proved to be independent risk factors of lymph node metastasis in rectal cancer. In gross type of the tumor, the risk of affecting LNM was descending in order of Knurl-type, Ulcer-type, Infiltration-type. It has great significance to forecast the risk of lymph node metastasis and formulate rational treatment from the research the factor- interrelated of lymph metastasis.
