Objective : To do the feasible research on the early definitive surgery in enterocutaneous fistulas patients on modern medical condition in our hospital. Methods:12 cases of enterocutaneous fistulas with early definitive surgery in our hospital from Oct.2006 to jan.2010 were reviewed.The age was (45.1±17.8) years old in this group.Among them,there are 11 men and 1 woman Enterocutaneous fistulas included 5 from duodenum, 5 from the small bowel, 1 from the ileorectal anastomotic dehiscence, and 1 from the pancreatic-colonic anastomotic fistula. The intestinal resection and anatomosis was performed in 6 patients and simple repaired in 6 patients. Results:Two patients died in this group,the other patients were cured and no new enterocutaneous fistulas was observed. 7 patients were sent to ICU after operation for (3.4士1.1)days.The time between early surgery and fistulas diagnosised was( 8.3±3.2)days. The time in hospital was( 34.5±15.1) days.The time of using somatostain was(9.1+6.9)days,9cases suffered from infection of incisional wound and 10cases suffered from bowel obstruction, 2 MODS cases happened,3 cases for lung infection,1cases for chronic disfunction of renal,and 2 cases for abdominal bleeding .Patients in this group got the average of the preoperative APACHE II score at (6.4±2.2)scores, and the average APACHE II score of the first day,the third day,the seventh day after operation were lower than the average of preoperation,they were respectively (4.3±1.8)scores,(4.1±2.0)scores and (2.8±1.5)scores, the differences had statistical significance(p<0.01),we thought the patients felt better after operation.Conclusion :With appropriate surgical techniques, ICU, nutrition support, organ function support and somatostain, somatotropin use, early definitive surgery can be performed safely and effectively for the enterocutaneous fistula patients according to the indication.
