Objectives:①By the retrospective analyse of the clinical cases,to find out the problems of treatment in the time of first-shoulder dislocation;②Based on the retrospective analysis of clinical cases,statistically analyse the data obtained from the questionnaire of the incidence of recurrent shoulder dislocation,propose preventive measures for decreasing the incidence of recurrent shoulder dislocation,and provide theoretical basis for developing more scientific training methods.Methods:①Directed to the cases of the soldiers diaglosed as recurrent shoulder dislocation at the Fuzhou General Hospital of Nanjing Military Region,during the period January 2000-December 2008,retrospectivly analyse the reasons for their injuries,damage types(eg,complications),treatments,fixed duration,rehabilitation,etc.②Based on the retrospective analysis of the clinical cases,design the questionnaire associated with the disease caused by military training (Attached tables),and then sent out 2910 copies of questionnaire to Military hospital out-patient,the military wards(including accompanys),a division of the Nanjing army stationed in Fuzhou City,and statistically analyse the data obtained.Results:1.Clinical Cases Review:(1)In the time of first-shoulder dislocation,the soldiers injured were not given enough fixation time,and lacked formal rehabilitation after the fixation removed;(2) There was such a situation that the injured soldiers were misdiagnosed in the time of first-shoulder dislocation and failed to make timely delivery to the higher attendance of the hospital;(3) In the time of first-dislocation,there were common features generalized as follow in the cases reviewed:the recruitment time of the injured soldiers,the reasons and training courses of injuries,degree of damage;(4)For cases with surgical treatment,there was significantly different shoulder function scores between the preoperative and postoperative group(p<0.05),the shoulder function was improved;(5)Although given the rehabilitation and oral Chinese medicine,two cases with conservative treatment both dislocated again within 3 months.2. Questionnaires:(1)The survey finds that the incidence of recurrent shoulder dislocation is 1.97%,recurrence rate of the shoulder dislocation is 82.8%;(2)The reasons for shoulder dislocation,training courses,time of injury obtained from the survey are almostly consistent with clinical cases reviewed;(3)The incidence of shoulder dislocation in the recruits is higher than veterans,particularly high in the recruits enlisted mainly in the first 3 months;(4)The duration of recruitment,fixing duration and rehabilitation in the time of first-shoulder dislocation are related to the recurrence rate of shoulder dislocation(p<0.05);(5)Veterans have much more understanding of the training preventive measures than recruits(p<0.05);(6)The mental qualities of recruits,the grassroots troops for training staff and medical staff's specialized knowledge play an important role in the recurrence of shoulder dislocation.(Note:The recruits are the soldiers joining the army in less than a year,more than a year are the veterans;)Conclusions:(1)The survey finds that the incidence of recurrent shoulder dislocation is 1.97%,recurrence rate of the shoulder dislocation is 82.8%.(2)The reasons for shoulder dislocation,training courses,time of injury that obtained from the survey are almostly consistent with clinical cases reviewed.(3)There is high incidence of shoulder dislocation in the recruits during training (particularly in the first three months of recruits-training).(4)The duration of recruitment,fixing duration and rehabilitation in the time of first-shoulder dislocation are related to the recurrence rate of shoulder dislocation(p<0.05).Inadquate fixing duration after the reduction and informal rehabilitation after fixation removed are the direct reasons for the recurrence of shoulder dislocation.(5)The mental qualities of recruits,the grassroots troops for training staff and medical staff's specialized knowledge play an important role in the recurrence of shoulder dislocation. It is needed to regulate the psychological factors of the recruits,instruct scientific training of the grassroots troops for training,consolidate and strengthen the professional knowledge and professional skills of the grass-roots medical staff.
